Wraparound Facilitator
(Permanent full-time, 35 hours/week)

Bargaining Unit Position

The Wraparound Facilitator position is responsible for providing Wraparound Services for youth
12- under 18 years old who are at risk of going into care or who are currently in care and require
support to return home. The position is responsible for guiding the family through a strengths
based process of Wraparound stages i.e. identifying and coordinating crisis/safety plans,
identifying and coordinating a collaborative team of supports, identifying family goals,
developing a customized plan, providing ongoing guidance and coordination of services,
advocacy, referrals and connecting to resources, ensuring the youth has a voice, and facilitating
the specific needs of the family in an effort to keep families together and help youth realize their
hopes and dreams.

Highlights of Your Role
Indirect client related activities/responsibilities include

Conduct outreach presentations to Child Welfare and Central Point of Intake to create awareness about Wraparound and generate referrals

Respond to inquiries, referrals, consults, determine eligibility – refer to alternate services if not eligible

Liaison/consultation/mediation with, service providers, team members, other stakeholders/relevant parties such as schools, counselling, parental supports, etc....

Advocacy

Coordinate with Child Welfare, and various service providers

Invite potential team members, explains process, secure commitment, and coordinate team meetings

Case consultation with internal colleagues i.e. Clinical consult, outreach worker, residential programs, etc...

Local travel to meetings/appointments

Research relevant programs and resources for clients

Facilitation/direct, client interaction activities/responsibilities include;

Promote/orient family to Wraparound, review/complete consents

Facilitate in-home, community, and virtual Wraparound meetings to address crisis or safety concerns, assess strengths, needs, culture, values, beliefs

Implement strengths based problem solving, identify creative solutions and coping strategies

Identify life domain areas to support family vision & goal

Identify/develop team of formal and informal supports

Develop, review, revise, record, and distribute Wraparound plans

Facilitate/participate in Family Conference meetings.

Meet one on one with youth to engage, build trust

Coordination, follow up, info-sharing via phone, or electronically

Accompany clients to meetings/appointmentsConduct pre/post evaluations and surveys

Admin duties/responsibilities include:

Completion of client and administrative reports, case notes, statistics, and client file management in accordance with organizational guidelines

Intake/create client file, upload relevant documents

Draft Wraparound plans, meeting agendas, strengths lists, and other Wraparound activity documents
