What We Do There has never been a more exciting time to be at WAHA. We are an integrated health system providing the full continuum of care to the six communities of the western James Bay coast with over 500 employees, physicians, and partners in care. As a health authority, you will work in a unique setting where you have the opportunity to care for patients in all phases of their life journey, in all areas of their health experience.   What You'll Do
  • Work in collaboration with the Healthcare Team to plan an effective discharge and community care process to ensure an easier transition back into the home environment.
  • Act as a resource and facilitator to discharge planning team, physicians, and staff.
  • Assist in the development of care plans when clients are back in their home settings.
  • Monitor clients progress while they are back in their home settings to determine if needs change over time.
  What You Bring
  • Ability to communicate in the local native Cree language is an asset
  • Diploma as a Social Service Worker is required
  • Bachelor of Social Work is strongly preferred
  • Registration in good standing with the Ontario College of Social Workers or a willingness to obtain is required
  • Relevant experience in discharge planning
  • Solid networking and outreach skills
  • Understanding of patient centred philosophy
  • Experience working with the senior's population an asset
  • Must possess a valid driver's license for Ontario

Salary: $58, $63,745.50
