We are an integrated health system providing the full continuum of care to the six communities of the western James Bay coast with over 500 employees, physicians, and partners in care. - Work in collaboration with the Healthcare Team to plan an effective discharge and community care process to ensure an easier transition back into the home environment.
- Act as a resource and facilitator to discharge planning team, physicians, and staff.
- Assist in the development of care plans when clients are back in their home settings.
- Monitor clients progress while they are back in their home settings to determine if needs change over time.
- Ability to communicate in the local native Cree language is an asset
- Diploma as a Social Service Worker is required
- Bachelor of Social Work is strongly preferred
- Registration in good standing with the Ontario College of Social Workers or a willingness to obtain is required
- Relevant experience in discharge planning
- Solid networking and outreach skills
- Understanding of patient centred philosophy
- Experience working with the senior's population an asset
- Must possess a valid driver's license for Ontario

Salary: $58, $63,745.50
