Make a meaningful impact on the lives of Albertans within chronic disease management as a team member of the Alberta Healthy Living Program. The community-based program supports adults living with or at risk of chronic health conditions, focusing on improving health and quality of life through patient-centered care. If you are a compassionate and enthusiastic Physiotherapist who enjoys working with adults with various chronic health conditions, this role may be for you. Under general supervision from the Care Manager, you will work as part of a multidisciplinary team to provide health coaching, supervised group exercise, education, and self-management support to help clients increase physical activity and adopt healthy lifestyle habits. Responsibilities include assessing clients, providing education, and supporting healthy aging and symptom management for conditions such as arthritis, osteoporosis, and chronic pain. The goal is to help clients exercise safely, restore or maintain movement, reduce pain, and improve overall quality of life. You will also lead group education on fall prevention, balance training, pain management, safe activity and exercise modifications, and the management of chronic neuro and musculoskeletal conditions. Services are provided both virtually and in-person at the East Calgary Health Centre and Thornhill Aquatic & Recreation Centre. This position works primarily 7.75 hours 3 days/week and 1/2 days 1 day/week. This position is part of Primary Care Alberta. Description:
As the Physiotherapist I (PT), you will provide therapeutic interventions to enable clients to achieve optimum physical function and independence in development, disease, disability, chronic conditions, illness and functional decline. You will provide intervention (i.e. prevention, triage, screening, assessment, diagnosis, goal setting, treatment, management, education, consultation, fostering resourcefulness, case management, referral and transition support to individuals, groups, populations and communities. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. The PT collaborates with patient(s) and families in determining, implementing and evaluating appropriate rehabilitation strategies. You will participate in a variety of interprofessional teams to build awareness, enrich supports and engage in quality improvement; partnering with healthcare providers, patients, families and other stakeholders to advance services for their clients. You will contribute to quality improvement and their professional community; participates in peer mentoring; and supervises students. You may provide clinical supervision to Therapy Assistants, Therapy Aides and Provisional Physiotherapists.

Required Qualifications:
Bachelor's degree in Physiotherapy from an accredited university program. Current registration with the College of Physiotherapists of Alberta.
Additional Required Qualifications:
Candidates must have at least three years within the past five years' experience providing patient-centered care for adults with chronic health conditions. This includes using active therapy and exercise prescriptions to support health and wellbeing. Must be proficient in leading group education sessions and offering individualized education to promote health, self-management, and overall well-being. The role requires expertise in fall prevention, pain and fatigue management, safe exercise modifications for osteoporosis, and managing chronic neuro and musculoskeletal conditions. Strong assessment skills are essential, including evaluating range of motion, muscle strength, and functional mobility. Experience in developing and implementing physiotherapy plans in collaboration with multidisciplinary teams is required. Excellent communication skills, including verbal, written, and listening, are essential, along with the ability to work both independently and as part of a team in a fast-paced environment. Candidates must be skilled at building respectful relationships with diverse clients, care providers, and team members, and have experience working with culturally diverse and vulnerable populations. Proficiency with computers, databases, and information systems is necessary. Physical ability to move equipment and lead cardio, strength, balance, and flexibility classes are also required. Must be able to travel between work sites during the day.
Preferred Qualifications:
Training and applied experience using Health Change Methodology or other chronic disease management, motivational interviewing, and health coaching strategies. Experience delivering virtual group education and one-on-one appointments. Experience using exercise science and prescription for adults with various chronic health conditions. Recent experience with virtual care using Zoom and Connect Care including documentation and workflows. Working knowledge of MS Word, Excel and Power Point.
