The Diabetes Educator (Registered Dietitian) for Specialty Populations works with high-risk clients and their families from specialty populations living with diabetes (pediatrics, pumps and pregnancy) and general adult clients living with diabetes. The focus of counseling clients and their families living with diabetes is to promote self-care, nutrition and lifestyle modification, and overall disease management with the goal to enhance quality of life, reduce the risk of complications, and to lessen the need for acute care services. The Diabetes Educator (Registered Dietitian) for Specialty Populations works independently providing education and assessment to clients, and collaboratively as an integral member of an individual's diabetes care team (composed of primary care providers, specialists, nurses, social workers and other allied health care professionals). This position reports to the Primary Care Network Manager (overall responsibility for this position including budget, performance management, etc.) and to the Provincial Diabetes Clinical Lead for diabetes clinical practice matters.

Duties will include but are not limited to:

  • Responsible for triaging complex high-risk patients referred by specialist physicians, primary care providers, general DEs, and PCRNs.
  • Responsible for determining scheduling needs for patients based on triage of complex issues, plan and need for follow up/education, responsible for the assessment, monitoring, and supervision of patients in the specialty setting.
  • Independent analysis, interpretation and assessment of insulin pump uploads and sensor data to guide treatment decisions.
  • For DEs specializing in pregnancy population, responsible for post-partum insulin delivery care plan.
  • Determines if clients meet and continue to meet the criteria to receive coverage through the province for the Insulin Pump Program.
  • Practices as a Registered Dietitian with advanced certifications and considered to be experts in their respective fields with the expectation that they will work autonomously.
  • Remains current regarding pertinent research and professional literature to provide the most up to date guidance and education to high-risk patients.
  • Conducts training on diabetes management for high-risk specialty populations (pediatrics, pumps and pregnancy) for health care professionals and personnel at schools (pediatrics).
  • Serves in a consultative and resource capacity for other health care providers.
  • Extensively supports the on-boarding of new clinical staff to the specialty service. On-boarding requires extensive mentorship, supervision and support over a period of two to three years as new staff learn competencies and complete the necessary certifications to independently practice.

Minimum Qualifications:

  • Bachelor's Degree majoring in Foods and Nutrition with a post-graduate internship in Dietetics or a Master's Degree in Community or Human Nutrition.
  • Certified Diabetes Educator (CDE) or requirement to obtain within 3900 hours of employment and re-certification as per the Canadian Diabetes Educator Certification Board requirements.
  • Health PEI certification in the Recommendation of Oral Diabetes Medications and Non-Insulin Injectable Diabetes Medications to be obtained within 1950 hours of employment.
  • Health PEI certification in Basic Insulin Dose Adjustment to be obtained within 1950 hours of employment with recertification every 5 years.
  • Health PEI Speciality Population Certifications (Pregnancy, Pumps, Pediatrics) to be completed in a stepwise manner and according to program needs, with the first certificate to be obtained within 2550 hours of employment.
  • Certified Pump Trainer as required by the program.
  • Requirement for continuing education, training and/or certification as per best practices and to meet new developments in equipment, technology, medications, etc. as required by the program.
  • Two years recent experience in a diabetes program setting, or 5 years recent experience in a related clinical nutrition setting (e.g., pediatrics, cardiac rehabilitation unit, renal unit) which included diabetes education and management.
  • Registration with the College of Dietitians of PEI or ability to obtain.
  • Excellent communication skills and interpersonal ability to operate in a collegial environment with a wide range of health professionals, institutions, agencies, and the public.
  • Ability to identify and analyze issues and make connections between issues. Able to independently take initiative to solve problems and make clinical decisions.
  • Ability to multi-task and prioritize competing demands.
  • Ability to effectively communicate verbally, in writing and electronically including the use of email, Microsoft Office, electronic medical record, and other related software.
  • Ability to work independently and as part of a team, while respecting flexible professional boundaries and team decisions
  • The successful applicant must provide a satisfactory criminal record check prior to beginning employment.

Other Qualifications:

  • Membership in Dietitians of Canada considered an asset.
  • Previous experience in diabetes education and adult education and/or pediatric education would be considered assets.
  • Experience working with individuals and providing group counselling would be considered an asset.
  • Experience with sensor-based technology and/or insulin pumps would be an asset.
  • Experience working with clients throughout different life stages (ie. pediatrics/ pregnancy).
  • Ability to engage with individuals, families, and groups at various stages of the disease and through various methods (in-person, telephone, email, virtual meetings).
  • Demonstrated skill for individual and group counselling.
  • Ability to organize, develop, implement, and evaluate education programs for clients, and health professionals.

Salary Range:
$ $46.70 per hour (Level 18)
