Position Summary
The Preventive Care and Telemedicine Registered Nurse (RN) is a key member of a regional program team, working collaboratively with primary health care teams to deliver both in-person and telemedicine services across the Kingston region. The RN provides community-based care within preventive screening, smoking cessation, and other preventive health programs for individuals and families.
In their role, the RN also facilitates virtual consultations by connecting clients with remote specialists, clinics, and programs using platforms such as the Ontario Telemedicine Network (OTN) and other emerging technologies. During these consultations, the RN conducts client assessments using tele-diagnostic equipment. The position also supports the growth of virtual healthcare services at partner sites as part of a broader regional strategy.
This role integrates clinical nursing responsibilities with administrative and system-navigation functions to ensure clients receive timely, coordinated access to preventive and virtual care.
Key Responsibilities
Telemedicine
- Provide all needed logistical support for telemedicine consultations (e.g., book consultations, reserve and prepare rooms, ensure availability of information/records, materials and equipment, operate videoconference and peripheral equipment and provide basic technical support/troubleshooting.
- Understand internal policies and processes with respect to organizational support systems for clinical events, i.e., medical records, admitting/registration, privacy, patient consent, etc.)
- Works in close collaboration with the interdisciplinary care team to support preparation of the patient and / or the Consultant for the clinical event, assisting with client presentation & examination as required.
- Ensure appropriate follow up, record keeping, statistics/reporting, and evaluation.
Preventive Care Screening
- Provide client-centered screening, consultation, assessment, and follow-up using a trauma-informed approach, recognizing social determinants of health and associated health risks.
- Develop holistic health prevention care plans and refer clients to internal and external service providers to support their health and wellness goals.
- Conduct intake assessments, referrals, case management, and follow-up as required.
- Collaborate with clients to assess goals and modify care plans accordingly.
- Provide health education and care in consultation with providers.
- Consult with other team members as needed to ensure safe and effective care.
- Serve as a leader and resource for internal stakeholders regarding cancer screening and preventive care related to health risks.
- Assist in the development of and adherence to medical directives as appropriate.
- Ensure infection control, occupational health, and safety standards are consistently followed in practice.
- Provide care in various settings, including clinic, rural community partner sites, mobile units (e.g., PORCH), virtual care, and home visits as appropriate.
- Facilitate access to and/or deliver programming related to preventive care, self-management, education, support, and resilience curriculum
(e.g., smoking cessation). - Participate in
preventive care screening and health promotion events within the community.
Service Delivery & Administration
- Collaborates with KCHC staff, partner agencies, hospitals, and other community settings as needed to deliver smoking cessation and other preventive care programs.
- Assists in creating and reviewing resource packages available for clients and adapts as necessary.
- Provides home visiting services to clients as needed.
- Completed up to date chart documentation in EMR for both individual and group encounters and support quality improvement initiatives by nursing interventions, referrals and recommendations for ongoing treatment.
- Participate and support additional quality improvement initiatives
- Engage in service/program development of the clinical telemedicine program, including liaising with healthcare providers and patients, and being a champion and advocate for the program.
- Support the development of clinical policies and procedures and implement clinical workflow processes.

Basic Education and Experience Requirements
- Registration in good standing with the College of Nurses of Ontario as a Registered Nurse (RN).
- Preferred 3-5 years of progressive clinical experience in a health care setting, including demonstrated experience working collaboratively with physicians and other allied healthcare professionals.
- Excellent health assessment and clinical skills appropriate for scope of practice, and the ability to provide clinical support for patients with a wide variety of needs.
- Basic understanding of the field of telemedicine or a willingness to learn.
Knowledge, Skills and Abilities
- Certified counsellor for Smoking Cessation is an asset
- Strong computer skills and willing to use new technology, including experience with electronic medical records.
- Comfort with new technology and proficiency in the use of computers and relevant software programs (e.g. Microsoft Office, Ncompass, PS Suites).
- Excellent communication (oral and written) and interpersonal skills, and a commitment to being an effective "team player".
- Problem solving skills, organizational skills, and attention to detail.
- Ability to, on a regular basis, lift up to 30 lbs, crouch and stand, and push and pull heavy equipment.
Other Requirements
- Valid driver's license, valid car insurance, daily access to a reliable vehicle, and willingness to travel within the program catchment area on a daily basis
- Current and satisfactory Criminal and Vulnerable Persons Check
- Current Immunization Record
