About the OpportunityWe are currently looking for a Social Worker to join our inter-disciplinary team of skilled professionals. As a member of the multi-disciplinary team in the Department of Patient and Family Centred Care, the Social Worker in this role will support the Community Stroke Rehabilitation program. The Social Worker will provide social work assessments and interventions to patients and families involved in the outpatient Community Stroke Program.
Regular working hours will be day shifts, Monday to Friday.

More About YouBasic Requirements:
- Bachelor's degree or Master's in Social Work, with relevant work experience.
- Current certificate of registration in good standing from the ON College of Social Workers and Social Service Workers
- Minimum three (3) years of experience working with patients who have had a stroke, other neurologic, complex chronic conditions and/or musculoskeletal disorders in a rehab or community setting is preferred
- Certification in Cognitive Behavioural Therapy, Motivational Interviewing, and Mindfulness-based stress reduction, preferred
- Completion of CorHealth's Interprofessional Stroke Core Competency Framework, or willingness to complete.
- Demonstrated comprehensive knowledge of community services, agencies, and placement facilities.
- Excellent communication skills and collaboration when interacting with patients, family members, and other members of the care team.
- Respect for individuals of diverse cultures and demonstration of culturally sensitive practice.
- Demonstrated effectiveness working with minimal supervision with proven reliability
- Ability to speak to a small group or on a one-to-one basis using appropriate vocabulary and grammar and the ability to speak and interact with persons of various social, cultural, economic, and educational backgrounds.
- Skill in writing concise, logical, grammatically correct correspondences and reports.
- Proficiency with personal computers, knowledge of word processing programs and associated software.
- Demonstrated satisfactory attendance levels, work performance, disciplinary history, and positive work references.
