Position Summary:

The Community Navigator will be the first point of contact for community members, providing essential support and resources. This role involves direct client interaction, data collection, and administrative assistance. Strong communication, empathy and professionalism are essential skills for this role.

The Community Navigator will provide care and resources to work with the most vulnerable to enhance the social, physical, mental and spiritual wellbeing of Indigenous peoples experiencing homelessness. They will aim to provide referrals to community agencies, light case management, build relationships with community agencies, provide naloxone training/kits to community, provide information regarding harm reduction, assist with recovery-based referrals, encourage elder consult and access to traditional medicines.

The Community Navigator will encourage individuals to pursue a path of healing through cultural knowledge and practices. It is vital that the best fit is made between the individual seeking support and the individual or individuals providing support. The Community Navigator will provide a bridge between community member and professional services. The AFCC Community Navigator will work closely with individuals to determine appropriate cultural practice(s), ensure cultural competency and build rapport in community.

Key Responsibilities:

  • Outreach for clients; first point of contact
  • Referrals to community supports and AFCC programming options
  • Collect data
  • Assist with other duties as required
  • Greet clients warmly and assist them with their initial inquiries.
  • Serve as the first point of contact for all clients entering the facility, providing a welcoming and supportive environment.
  • Assess clients' needs and refer them to appropriate community support services and programming options.
  • Provide information on various resources available to clients, ensuring they receive the assistance they need.
  • Accurately collect and record client data for program evaluation and reporting purposes.
  • Ensure all client information is handled with confidentiality and care.

Qualifications:

  • Knowledge with other community agencies, programs and resources.
  • Experience working with vulnerable populations or in a community support setting is an asset.
  • Excellent communication and interpersonal skills.
  • Strong organizational skills and attention to detail.
  • Ability to handle sensitive information with confidentiality and professionalism.
  • Proficient in basic computer applications (e.g., MS Office).
  • Ability to work independently and as part of a team.
  • Empathetic, patient, and respectful towards clients from diverse backgrounds.
  • Problem-solving attitude and a proactive approach to tasks.
  • Flexibility to adapt to changing priorities and tasks.

Hours of Operations:

Seven days per week

  • Monday – Friday 9:00 am – 4:00 pm
  • Saturday – Sunday 11:00 am – 4:00 pm
