**Position Summary**: The Patient Navigator plays a critical role in supporting clients as they move through Shkagamik-Kwe Health Centre’s primary care and integrated health services. Reporting to the Director of Primary Care, the Patient Navigator supports patients in understanding, accessing, and coordinating health and social services in a culturally safe, respectful, and client-centred manner.This role acts as a consistent point of contact for patients, helping to reduce barriers to care, improve continuity, and enhance the overall patient experience. The Patient Navigator works closely with interdisciplinary care teams, internal programs, and external service providers to ensure clients receive timely, coordinated, and appropriate supports.
**Key Responsibilities**:
- Serve as a primary point of contact for patients navigating SKHC primary care and related services
- Support patients in understanding care plans, referrals, appointments, and follow-up requirements
- Assist patients in accessing internal programs and external health, social, and community resources
- Identify and help reduce barriers to care (e.g., transportation, system navigation, appointment coordination)
- Coordinate and follow up on referrals to internal services and external providers
- Maintain accurate, timely, and confidential documentation in electronic medical records and tracking systems
- Communicate effectively with physicians, nurse practitioners, nurses, allied health professionals, and administrative staff
- Advocate for patient needs while respecting scope, professional boundaries, and clinical decision-making
- Support culturally safe care delivery that aligns with SKHC values, teachings, and community priorities
- Participate in team meetings, case conferences, and quality improvement initiatives
- Support continuous improvement of patient navigation processes and workflows
- Uphold privacy, confidentiality, and ethical standards at all times
**Qualifications & Experience**:
- Diploma or degree in a health, social services, or related field (or an equivalent combination of education and experience)
- Experience working in health care, community services, or client-facing support roles
- Strong understanding of health system navigation, referrals, and community-based services
- Experience working with diverse populations; experience in Anishinaabek health settings is a strong asset
- Excellent communication, interpersonal, and organizational skills
- Strong attention to detail with the ability to manage multiple priorities
- Demonstrated ability to work collaboratively within interdisciplinary teams
- Proficiency with electronic medical records and standard office software
- Ability to maintain confidentiality and exercise sound judgment
