Crosstown Family Health Team (CFHT), in partnership with the Weston Family Health Organization (FHO), provides comprehensive primary health care services, including chronic disease management, health promotion, and disease prevention, to a rostered population in the York South–Weston area, including Mount Dennis.

As a not-for-profit organization governed under the Ontario Not-for-Profit Corporations Act, 2010 (ONCA), we are committed to excellence in patient care and community well-being. Our inter-professional team of health care providers works collaboratively to deliver accessible, high-quality care to our community.

We are currently seeking enthusiastic and committed volunteers to join our Board of Directors.

About Crosstown Family Health Team

Crosstown FHT is a dynamic, community-based primary care organization dedicated to improving the health and well-being of residents in York South–Weston and Mount Dennis.

Through an inter-professional model of care, we work to ensure that every patient has access to comprehensive, coordinated, and person-centred services. Our programs emphasize prevention, health promotion, chronic disease management, and collaboration across the health system.

Board Member Eligibility and Expectations

To be eligible to serve as a Director, candidates must:

● Be 18 years of age or older

● Support the Vision, Mission, and Values of Crosstown FHT

● Support and uphold the Strategic Plan

● Be able to speak and comprehend English well enough to participate fully in Board discussions and materials

● Have no undischarged bankruptcies

● Have no conflicts of interest

● Be able to attend meetings and review Board materials in advance and contribute informed expertise at meeting

● Serve as an advocate for the organization and help advance its strategic priorities through community engagement and partnerships

Preferred Skills and Experience

Candidates with experience or education in any of the following areas are encouraged to apply:

● Governance and strategic leadership

● Financial management or accounting

● Legal expertise

● Information technology and digital health

● Dispute resolution or mediation

● Community relations and engagement

● Capital planning and facilities development

● Clinical program development or health care management Commitment

● Minimum one-year term, with eligibility for re-election

● Directors are appointed by the Members at the Annual General Meeting or as vacancies arise

● The Board meets quarterly (September, December, March, and June), with an Annual General Meeting (AGM) held in June

● Each meeting is approximately two hours, with an estimated two to four additional hours per month for review or committee work as needed

● Meetings are currently held virtually, with hybrid or in-person options available going forward
