Job description
We are looking for a qualified Registered Nurse-(RN), and Registered Practical Nurse-(RPN) to care for our clients professionally and compassionately. Assuming the responsibility of the Staff (RPN), It's often a demanding job as you will have to be flexible for phone support during the day and after hours. But, it can also be very gratifying. People who are ill, injured, disabled, or elderly are deserving and thankful for the exceptional support.
The ideal candidate reports to the Managing Director. The RN/RPN will provide professional nursing care to clients and monitor the care given by staff members. He/she will be required to assess, plan, and direct treatment for the Care Plan and work closely with the Home-Care-Response team. Be proactive and engaging with outstanding communication skills. You may be eligible for standby pay and flex time work, and should be a leader that can manage caseload.
Assessment:
- Monitor and provide support to nursing staff and support RPNs as needed.
- Complete client assessments as required for new business care plan and safety, quarterly and on change in health status in accordance with policy and standards of Home Care.
- Initiate, co-ordinate, assess and evaluate staff reports for care.
- Ensure that outcomes of all assessments are communicated to the appropriate manager as required for further evaluation.
- Alert Public Health in the event of a suspected outbreak.
Care Planning:
- Monitor the care provided and updating care plans as required.
- Maintain accurate and complete records of medication and nursing care delivery.
- Communicate the plan of care to the client/substitute decision maker ensuring that treatment decisions are informed and respectful to the their expectation.
- Have a professional working knowledge of all equipment used in the delivery of nursing care and ensure the safe use of same by other members of the team.
- Ensure care plans provide Level of Care and accurately documented by the team.
- Complete assigned client quarterly nursing summaries on schedule.
Requirements:
- Current Certificate from the College of Nurses of Ontario verified upon date of hire & reviewed annually.
- Skill in planning, co-ordinating and leading other nursing personnel and support service staff.
- Proficiency in interpersonal communication skills, both written & verbal.
- Computer skills: has a working knowledge of Microsoft word and keyboarding skills.
- Must have a valid driver's license and access to a vehicle
- Ability to speak a second language is considered an asset
- Excellent knowledge of emergency response & First Aid (CPR)
- Good time management skills
- Clear Police Clearance.
