The Quality Assurance and Specialized Services Team is looking for a qualified Transitional Age Youth Coordinator (TAY Coordinator). The TAY Coordinator provides expertise to individuals, families and caregivers looking to transition from children/youth services to Adult services. The TAY Coordinator engages multiple stakeholders within the identified circle of care team to ensure a smooth transition from children's services to Ministry of Children, Community and Social Services (MCCSS). Supports include regular communication and check in meetings with service users and service providers to ensure identified goals are met and adjusted to changing support needs, administrative needs, identified budget and individual's goals. The TAY Coordinator regularly coordinates with agency partners including Outside Paid Resources (OPR) and Transfer Payment Agencies (TPA) to explore potential residential/non-residential adult supports and services in line with the supported person's goals. The TAY Coordinator is responsible for developing the initial individualized budget for supported person's transition to adult services as well as ongoing correspondence with OPRs, TPAs and circle of care team regarding transition status, funding approval and additional ministry directions or updates. The TAY Coordinator engages in ongoing discussion and collaboration with the Quality Assurance Team, identified circle of care as well as external stakeholders to ensure individualized budget meets service user needs as identified in individualized plan.
Under the supervision of the Program Manager and receiving support from the Team Lead, key accountabilities include:
- On behalf of Extended Society Care (ESC) or Complex Special Needs (CSN) youth, 14 to 18+ years of age, transitioning from children's services and referred within Durham, Haliburton, Kawartha Pine Ridge (HKPR) regions, the Coordinator provides transition planning services for youth requiring supportive adult services within the province of Ontario.
- Prepare the individual, families and systems for transfer to Ministry of Children, Community and Social Services (MCCSS) funded supports and services where planning is individualized, holistic, flexible, strength-based and culturally appropriate.
- Ensure transition planning processes and correspondence are documented for each individual, with regular communication among involved agencies and individuals during the transition period to adult services.
- Provide education and information to the youth and their circle of care team on community funding sources and application processes including; Ministry of Children, Community and Social Services (MCCSS), Developmental Services Ontario (DSO), Ministry of Health, Ministry of the Attorney General and Ontario Disability Support Program (ODSP).
- Adapt plans to accommodate or respond to changes for individuals accordingly.
- Act as the main contact of communication between MCCSS and Developmental Service Agencies/Outside Paid Resources' (OPR's) regarding budget submissions, budget clarifications and budget tracking.
- Responsible for ongoing correspondence with OPRs, TPAs and circle of care team regarding transition status, funding approval and additional ministry directions or updates.
- Ensure supporting documentation for Disability Services Ontario application is completed.
Education, Training and Experience:
- Post-secondary Degree in Social Sciences, Social Work or related field or equivalent.
- Registered Social Worker designation is considered an asset.
- 3 years of experience in Case Management experience.
- 3 years of experience of direct support in a residential setting.
- Demonstrated experience of using best practice in supporting individuals with an intellectual disability.
- Annual completion of a Criminal Background Check/Declaration, including Vulnerable Sector Screening (VSS).
- First Aid CPR Level C.
- Non-Violent Crisis Intervention Training / Safe Management.
- Valid G-Class driver's license for the Province of Ontario, access to reliable transportation and have valid proof of automobile insurance with driving for business purposes and a minimum liability of two million dollars.
