Hospice Palliative Care Ontario (HPCO) is a provincial association for hospices and palliative care providers, professionals, and volunteers throughout Ontario. HPCO has an opening for a Nurse Educator to develop, deliver and evaluate educational offerings to increase palliative care knowledge and capacity across the province. Reporting to the Director, Centre for Excellence, the Nurse Educator is responsible for achieving annual goals and objectives for the palliative care education program for long-term care homes and community support services in the Toronto region as well as for assigned province-wide education programming.

Key accountabilities will include, but will not be limited to:

  • Consult with leadership and conduct needs assessments of service provider organizations to understand palliative care learning needs, with a focus on long-term care homes and community support services within the assigned geography.
  • Develop, deliver and evaluate palliative care education programming, including in-person and through synchronous and asynchronous virtual media.
  • Assist service provider organizations in the application of assessment tools and best practice guidelines.
  • Develop and maintain relationships with key contacts at long-term care home and community support services organizations.
  • Develop and implement mentoring programs across sectors.
  • Provide program reports and statistics as required.
  • Identify funding opportunities and develop grant applications.

Key Qualifications:

  • BScN with RN registration in good standing from the College of Nurses of Ontario. A graduate degree is an asset.
  • CHPCN(C) required. Other completed palliative education (e.g. CAPCE, LEAP, Fundamentals of Palliative Care) is an asset.
  • Minimum of five (5) years of experience in palliative care, including supervisory experience in a community palliative care setting (long-term care, hospice or home care).
  • Knowledge of the provincial Palliative Care Program, Ontario Palliative Care Network and the Palliative Care Health Services Delivery Framework in the Community, the Ontario Palliative Care Competency Framework and Health Quality Ontario's Palliative Care Quality Standard.
  • Knowledge of relevant legislation and requirements including the Fixing Long-Term Care Homes Act and the Compassionate Care Act.
  • Effective consultation skills.
  • Teaching, mentoring and coaching experience.
  • Experience working with a range of roles, disciplines and levels of staff in different health care settings, including community palliative care.

This position is hybrid - Work from Home and onsite at long-term care and community support services organizations in the Greater Toronto Area. We are open to flexible position types, including full-time or contract.
