Position Summary

Social Worker, Adult Mental Health Program – Day Hospital
As a Social Worker, you will be a fully integrated member of an interdependent multidisciplinary team. You will be part of the team responsible for providing individual, family and group therapy using Best Practices in the program. You have the ability to work independently and, in a team setting, to set priorities, organize and coordinate activities. You have individual, family and group therapy experience and demonstrated skills in program development, implementation and evaluation and in keeping statistical records. You have the demonstrated clinical ability to broker and coordinate community resources. You have skills in CBT, DBT, Motivational Interviewing and Brief Therapies. You also have excellent listening and communication skills, both verbal and written. This position supports the Hospital's academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for staff, physicians, volunteers, learners, patients and families.



On a practical level, you will


• Work in accordance with regulatory standards and professional practice guidelines, policies and procedures for both North York General Hospital, and the Ontario College of Social Workers and Social Service Workers.

• Demonstrate knowledge and experience in individual and group therapy modalities.

• Perform Social Work duties as required to meet the needs of the patient/client population

• Maintain an updated knowledge of community resources, identify gaps in service and respond to client and community needs.

• Offer group, individual or family counselling where appropriate.

• Together with the team, guide patients in the development of a crisis plan that supports their wellness and recovery while attending day hospital and at discharge

• Demonstrate the ability to collaborate effectively as a member of the health care team and take initiative and leadership when appropriate.

• Participate in program planning, implementation and evaluation

• Participate in the development and maintenance of effective linkages with Hospital and community service providers.

• Participate in the education of staff, students and the community



Qualifications


• Current registration with the Ontario College of Social Workers and Social Service Workers

• Master of Social Work or equivalent

• Extensive knowledge of local community mental health services is an asset

• Clinical experience using CBT, DBT and other relevant clinical modalities are an asset

• Excellent Clinical skills

• Membership in good standing or eligibility for membership in the Ontario College of Social Workers and Social Service Workers or equivalent

• Minimum 3-5 years of mental health experience. Experience in Mental Health Outpatient or Partial Hospitalization setting will be an asset.

• Membership with the Ontario Association of Social Workers strongly encouraged

• Demonstrate competence to perform the Controlled Act of Psychotherapy
