ABOUT THE ROLEWe are seeking highly dedicated and professional Licensed Practical Nurses (LPN) to join our health and wellness team The Licensed Practical Nurses will work to their full scope of practice in providing safe, quality care in accordance with established standards, policies and procedures of the FMFN Community Health and Wellness Department. The candidates must demonstrate the ability to be independent, show attention to detail and provide patient oriented service.These are Casual positions reporting directly to the Riverside Care Facility Nursing Supervisor. Available hours vary based on client and departmental needs. Day shifts range from 8 to 12 hours and 12-hour nights. Transportation is provided to and from work. This position is based in Fort McKay, AB.KEY RESPONSIBLITIESResponsibilities include, but are not limited to:Perform and complete delegated independent nursing functions using established procedures, policies, guidelines, and facility standards.Utilize nursing knowledge and skills implementing basic preventative therapeutic and rehabilitative nursing care of assigned patient/familyAssesses, plan, implement and evaluate individualized care to meet the residents' physical and emotional needsCommunicate with relatives regarding the care and condition of respective residents as requiredLiaise with attending physicians, physiotherapist, social workers, relatives or responsible persons, nursing, dietary, activity staff, etc. to ensure effective resident care is providedMaintain and coordinate communication between patient/significant other, staff, and physician as evidenced within medical record, client, and health care team feedback.Give timely change of shift report consisting of pertinent and relevant information for assigned patients.Administer medication accurately, observing patient response, as evidenced by documentation in the medical record.Perform limited I.V. therapy, and monitoring on those patients receiving peripherally inserted intravenous fluids, as delegated by and observed by the RN Case WorkerProcure and administer medications including injectionsCheck and order inventory supplies; maintain facility equipment's, order replacement supplies as needed.Collect lab specimen as per Doctors orders.Provide basic bedside nursing care, taking vital signs such as temperature, blood pressure, pulse, and respiration.Dress the patients' wounds regularly to promote fast healing and taking care of body's sanitary requirements.Monitor food and liquid input/output.Prepare and give injections and enema, monitor catheters, apply dressings, and treat bedsores.Monitor patients and report adverse reactions to medications or treatmentsConduct home visits to build relationships with community members and provide information and support, as required.Assist with providing health promotion and injury prevention services such as prenatal classes, health fairs, or teaching, as required.Refer clients and families to appropriate resources and services and help them overcome any access barriers.Participate in the investigation of communicable disease outbreaks and provide information, screening and follow-up.Other duties shall be assigned as required.KEY REQUIREMENTSGraduate of an Accredited Licensed Practical Nurse (LPN) Education ProgramCurrent and valid practice permit with CLPNA – experience in long-term care an assetCurrent Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) is required.Valid First Aid/CPR Health Care ProviderMinimum 2 years of working as an LPN in a continuing care facility preferredA sound knowledge and background in home care nursing preferredWound Care knowledge an assetSupportive Pathways/P.I.E.C.E.S.
