Home and Community Care Nurse

PROJECT SUMMARY

The Lillooet Tribal Council is recruiting a Full-Time Home and Community Nurse for the St'át'imc Outreach Health Services (SOHS). The ideal candidate has a strong understanding of the unique challenges faced by Indigenous clients and are committed to delivering compassionate, yet empowering care.

The Home and Community Care Nurse reports to the Clinical Team Lead. This is a full-time permanent position, with the possibility of considering part-time applicants/flexible work schedules.

The Nurse works closely with individuals and families, supporting their overall wellbeing by offering guidance and oversight to Community Home Support Workers. Responsibilities include developing wellness plans, enhancing health literacy, and addressing any barriers to ensure ongoing culturally sensitive care.

KEY RESPONSIBILITIES

• Establish a trusting and culturally safe communications with clients and families.
• Develop relationships with community staff, colleagues, and external agencies.
• Provide direct nursing care to support community home care services.
• Complete client assessments, including medical history, medications, symptoms, family/ friends support, and treatment plans prescribed by other health professionals.
• Work with clients and families to develop a wellness plan for case management needs.
• Support clients with understanding their wellness plan, including their illness, diagnostic tests, medications, procedures, etc. towards making informed decisions.
• Assist clients with referrals to external resources and coordination of services as needed.
• Support discharge planning with appropriate service providers.
• Provide education to clients, families, etc. to ensure informed wrap around care.
• Deliver workshops and presentations on health and prevention.
• Prepare reports on program status and community health trends.

QUALIFICATIONS

• A minimum of 5 years' relevant experience.
• Bachelor of Science in Nursing (BSN), Registered Nurse, (Licensed Practical Nurses/ LPNs will be considered based on experience).
• Current registration and in good standing with the British Columbia College of Nurses and Midwives (BCCNM)-  
• Knowledge of available community resources and how to access them.
• Cultural competency and safety when working with the St'át'imc.
• Successful completion of a Criminal Record Check.
• Valid driver's license.

SALARY: The wage range for the Home and Community Care Nurse position is based on the BCNU Wage Grid and is structured to align with our specific budget constraints. This range aims to provide competitive compensation while remaining adaptable to variations in experience and qualification.

• Registered Nurse (RN): $41.42/hour to $43.00/hour
• Licenced Practical Nurse (LPN) qualified applicants: $32.84/hour

SCHEDULE:

• Standard 7-hour shift, Monday to Friday, with flexible scheduling options available (Evenings and weekends as needed).
