POSITION SUMMARY
Reporting to the Health Manager, the Primary Care Nurse is responsible for providing comprehensive and coordinated nursing care within a community-based, interprofessional team environment. The Primary Care Nurse practices in accordance with the standards of professional practice and code of ethics as outlined by the British Columbia College of Nurses and Midwives (BCCNM). This position delivers client and family-centered care through assessment, planning, implementation, and evaluation of health interventions that promote wellness, disease prevention, and management of chronic and acute health conditions.
The Nurse provides care across settings—including clinics, homes, outreach, and virtual environments—and ensures clients and families have access to timely, holistic, and culturally safe health services. The position applies a trauma-informed, equity-based, and culturally respectful approach that honours Indigenous knowledge and promotes self-determination, dignity, and independence for all community members.

EDUCATION/EXPERIENCE

•Graduation from an approved School of Nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM)

•Current CPR – Basic Life Support certification

•Current valid BC Driver's License

•Experience providing community-based nursing services in primary care or public health settings preferred

•BCCNM Certified Practice (RN First Call, Remote Nursing, Sexually Transmitted Infections, and/or Contraceptive Management) considered an asset

•Valid BC Driver's License and acceptable Driver's Abstract

•Acceptable Criminal Record Check with Vulnerable Sector Search

•Demonstrated ability to maintain confidentiality, sound judgment, professionalism, and discretion in all interactions

• Ability to travel within the community and surrounding areas as required

JOB DUTIES/RESPONSIBILITIES

•Completes comprehensive health assessments addressing the physical, emotional, social, spiritual, and cultural needs of clients and families

•Determines urgency of health concerns, triages clients appropriately, and refers to other health professionals or programs as required

•Provides direct clinical care and nursing interventions within professional scope, including health promotion, disease prevention, chronic disease management, and urgent care support

•Collaborates with physicians, nurse practitioners, allied health professionals, and community partners to develop, implement, and evaluate care plans aligned with client goals and needs

•Establishes and maintains therapeutic, respectful, and trusting relationships with clients and families through trauma-informed and person-centered communication

•Provides health counselling, education, and self-management support to individuals and families in clinic, home, and outreach settings

•Coordinates care transitions across primary, community, and acute care systems to optimize client health outcomes and reduce hospital admissions

•Facilitates access to and navigation of the health care system, ensuring culturally safe and seamless client experiences

•Participates in outreach nursing care, home visits, and community wellness initiatives, including chronic disease prevention, sexual health, maternal-child health, and elder care

•Promotes wellness, independence, and health literacy through education, group sessions, and individualized teaching

•Documents assessments, interventions, and outcomes in electronic and paper records to ensure continuity and quality of care

•Participates in program planning, evaluation, and continuous quality improvement initiatives within the Health Department

•Adheres to safe work procedures and infection control standards, reports safety incidents or equipment issues in accordance with policy

•Supports orientation, mentoring, and knowledge sharing with nursing students, new staff, and interprofessional team members

•Participates in professional development, research, and continuous learning to enhance nursing practice and program delivery

•Advocates for clients to protect and promote their rights to dignity, respect, privacy, autonomy, and access to information

•Performs other related duties as required and in accordance with Westbank First Nation policies, standards, and procedures

KNOWLEDGE/SKILLS/ABILITIES

•Proven ability to conduct health assessments and provide appropriate nursing interventions based on clinical reasoning

•Strong interpersonal and communication skills, with the ability to collaborate effectively within interprofessional teams

•Demonstrated critical thinking and decision-making skills, integrating data and evidence-based practice into client care

•Commitment to person- and family-centered care that is culturally responsive and trauma-informed

•Demonstrated teaching and facilitation skills with individuals, families, and groups

•Effective organizational and time management skills to coordinate multiple priorities in a fast-paced environment

•Demonstrated leadership abilities in promoting teamwork, collaboration, and positive change within the workplace

•Proficiency in Microsoft Office applications and electronic medical record systems

•Physical ability to perform the duties of the position

•Understanding of Indigenous health determinants, local Syilx culture, and the importance of culturally safe service delivery
