About the Role

A comfort to our residents and a leader to the team – being a Clinical Care Coordinator is all about how you make people feel.

Estia Health Hamilton has an opportunity for an experienced Clinical Nurse/CCC with previous supervisory experience to step into this role. Options to work either Thursday to Monday or Friday to Tuesday.

You will:

  • Drive a continuous improvement culture
  • Acting as a clinical resource for staff, offering clinical education
  • Focusing on clinical service delivery, compliance, and audit processes
  • Care planning in line with new standards
  • Taking pride in your clinical documentation management skills

You'll be someone who is empathetic and who's previously worked in an aged care home. You'll also have:

  • A real desire to make a difference in people's lives
  • Leadership experience in clinical care
  • A nursing degree and current AHPRA registration
  • Solid documentation skills and knowledge of ANACC
