Nurse Practitioner

Full-Time Contract Position, For a Period of up to 14-months

Durham Medical Clinic, Durham ON

This position provides an exciting opportunity join a dynamic team of local physicians and interdisciplinary health care professionals at our Durham Site.

Main Responsibilities:

  • Work collaboratively with physicians and inter-disciplinary team members to effectively manage patient needs.
  • Provide a full range of primary care services including assessment, diagnosis, treatment, follow-up, education, consultation, and referral.
  • Deliver health promotion and disease prevention management and support.

Position Qualifications and Preferred Experience:

  • Bachelor's degree in Nursing.
  • Current Certificate of Registration in the Extended Class from the College of Nurses of Ontario, and eligibility to practice as a RN-EC.
  • Experience practicing as a NP in a Community Health or Primary Care setting.
  • Proven ability to communicate effectively with patients, families, and members of an interdisciplinary team.
  • Experience in delivering patient education and chronic disease management.
  • Proficiency in the use of computers and healthcare information technology.
  • Possession of a valid driver's license and access to a reliable vehicle.
