Overview

CMHA Durham is an integrated community mental health and primary care hub that has been providing mental health services to Durham Region for over 60 years. The organization offers a broad range of services through inter-professional teams focused on clinically based services that address social determinants of health using the recovery model.

The Nurse Practitioner provides services to complex clients throughout the lifespan, the client family, and the community, with a strong emphasis on health promotion, patient education and disease prevention in a psychiatric and addictions treatment setting. The role includes participation as a member of a high-performance, multi-service team of Case Managers, nurses and other health specialists providing Primary Care to clients within the HART Hub residential setting.

Duties and Responsibilities

- Assessment: Within the scope of the RN(EC), perform comprehensive health or symptom-directed assessments, synthesizing data from multiple sources to aid diagnosis as part of a comprehensive health care team.
- Systematically assess clients to identify existing or potential health problems, focusing on lifestyle, psychological, sociocultural and environmental risk factors; provide individualized care or refer to/consult with other health care providers.
- Encourage clients and families to participate in risk factor and health problem identification, goal setting, and intervention choices for disease treatment, prevention, and health promotion.
- Work within medical directives established and approved by CMHA Durham and collaborating physicians.
- Determine, order and interpret diagnostic imaging results within the RN(EC) scope of practice.
- Maintain complete, accurate, legible and timely records of client visits to support ongoing care among practitioners.
- See clients for scheduled and urgent assessments across the continuum of need, during regular and extended hours in all service areas (including MUP).
- Identify and respond to urgent/emergent health issues in collaboration with the multidisciplinary team.

- Treatment: Provide advanced nursing care and treatment (including pharmacological, complementary therapies and/or counseling interventions) for complex client conditions in accordance with best practices and within the RN(EC) scope of practice.
- Monitor efficacy and adjust the clinical care plan in collaboration with the multidisciplinary team; document in client health records and arrange follow-up as necessary.
- Implement strategies to promote healthier lifestyles and involve clients in decisions about their own health.
- Synthesize client information to identify broader social implications; ensure encounter forms and statistics are recorded and analyzed; participate in quality improvement efforts.
- Provide health education across the continuum of care with a focus on mental health and addiction recovery.

Organizational Role

- Use community assessment data to identify needs and resources to guide programming; stay current with organizational policies and meetings.
- Contribute to quality performance and achievement of organizational goals; manage priorities and adhere to established procedures.

Client Safety

Adhere to CMHA Durham client safety programs and infection prevention procedures. Maintain open communication with clients, families and staff to achieve quality safety outcomes. Report client safety incidents and suggest improvements to management.

Other Duties

- Attend team meetings and organizational meetings; participate in quality improvement activities, accreditation projects, committees, and events.

- Regularly sit, stand, and walk; concentrate and be attentive to assess clients, administer medications, prepare documents, listen to clients and operate nursing equipment; assist with mobility aids as needed.

Minimize risks by adhering to CMHA Durham policies and procedures and by identifying, assessing, evaluating and controlling risks to ensure well-being of employees and clients.

Qualifications

- Master's in nursing or a health-related field; Bachelor of Science in Nursing; Current registration with the College of Nurses of Ontario as an RN(EC).
- Current membership with the NPAO; Ontario Driver’s License is an asset; experience with electronic medical records is an asset.
- Knowledge of Mental Health Act, Health Care Consent Act and related Ontario legislation is an asset; knowledge of psychotropic medications and ability to manage medication checks.
- Ability to develop and maintain positive relationships with clients and community partners; professional representation of the organization.
- Excellent communication, assessment, organizational skills and ability to work with diverse populations; flexibility to adapt to change and support functions within the clinic (injections, phlebotomy, admin support); ability to work independently and as part of a high-performance team.
- MS Office and internet proficiency; knowledge of Indigenous communities and cultural practices (preferred); intermediate French is an asset; ability to work outside normal hours as required.

Working Conditions

- Works in a low-barrier community treatment environment; exposure to clients in crisis and to hazards such as blood/bodily fluids, cleaning fluids, needles and medications.
- Possible exposure to unpredictable psychiatric behaviors, deadlines, and multiple demands; environment may require work outside normal hours per policy.
