About the Role

Location of work: Centretown Community Health Centre / Somerset West CHC and surrounding catchment areas.

The ANCHOR Team is an innovative pilot project that will provide a community-based, non-police response and follow-up for individuals experiencing crises in the Centertown and Somerset West catchment areas. The project is a collaborative initiative. Partners include Centre town and Somerset West Community Health Centre (SWCHC), the City of Ottawa, the Ottawa Guiding Council on Mental Health and Addictions, Community Navigation of Eastern Ontario (CNEO) and other community stakeholders.

Reporting to the Manager, ANCHOR Team, and the Post-Crisis Support Worker (PCSW) works with clients referred by ANCHOR Crisis Response Workers to support them to further stabilize and connect with internal and community resources in the immediate aftermath of a mental health or substance use crisis. The incumbent does this through assessment, short term supportive counselling, advocacy, information gathering, referrals and guides individuals in navigating systems. The PCSW role is designed to work with clients for up to 90 days after crisis intervention.

The PCSW is based out of Centretown CHC but will conduct activities primarily in the community.

The PCSW will work with a wide range of stakeholders to enhance community engagement and networks to better support clients to navigate health and social care systems in the community.

Requirements
  • Master's degree in Social Work, or equivalent combination of training, professional and lived experience.
  • Minimum of three (3) years' experience in counselling, case management and/ or post-crisis support.
  • Solid knowledge of mental health, trauma, substance use, addictions and harm reduction
  • High sensitivity to psychosocial, social, mental health and health needs of clients.
  • Extensive knowledge of health and social services system/resources in Ottawa.
  • Training and education in trauma-informed approaches.
  • Strong documentation skills, proficient with technology including desktop and mobile platforms.
  • Crisis intervention and de-escalation skills.
  • Experience with a wide range of groups with high sensitivity to equity issues
  • Excellent written and verbal communication skills.
  • Ability to flex schedule when required to respond to needs of a 24/7 program
  • Ability to work well independently and as part of a multi-disciplinary team.
  • Driver's License (G class) required to operate motor vehicle
