Position Summary:

The position of Covenant Living Model Support Coordinator will commit to incorporate our mission, vision, and our values, which are Compassion, Respect, Collaboration, Social Justice, Integrity and Stewardship, and uphold our ethical traditions as foundational aspects in all actions and decision-making.

With the functional support, guidance and supervision provided by the Covenant Living Manager, 2 temporary Support Coordinators will carry out services as outlined by the Covenant Living Policies and Procedures in all Alberta locations.

The Support Coordinators will be responsible to provide information, education, care coordination, and care navigation to support clients and their caregivers who are participating in the Covenant Living project. Connecting Covenant Living clients with appropriate supports and services, the 2 incumbents will function independently in both formal and informal health care environments, in accordance with Covenant Mission, Vision and Values

This position will:

  • Model the organizations mission, vision, and values, consistently representing Covenant professionally and positively.
  • Collaborate effectively as a professional team plater, supporting the achievement of Covenant's Mission, Vision, and Values.
  • Engage and mobilize teams to fulfill Covenant's Mission, Vision, Values, and strategic objectives.

The Covenant Living Support Coordinators will be accountable for:

  • Clinical Practice
  • Stakeholder Engagement

Qualifications:

  • Bachelor's degree in social work required
  • Active registration with the Alberta College of Social Workers (ACSW)
  • Minimum of 5 years of experience in a community or clinical senior care setting providing assessment, information, referral, system navigation/case management services
  • Knowledge of the community-based senior serving sector in Edmonton
  • Experience in service development and community engagement
  • Experience with electronic documentation systems
  • Strong ability to build relationships with a diverse range of stakeholders
  • Strong verbal and telephone/virtual communication skills
  • Strong writing and documentation skills
  • Strong adult education skills
  • Strong literature review skills
  • Ability to function in a fast-paced environment with interruptions and changing priorities
  • Ability to work within a group as well as independently
  • Flexibility in scheduling
  • Ability to respond to a variety of questions and adapt to the changing needs of the client
  • Ability to individualize client care and educational needs

This is a temporary, full-time position for approximately one (1) year.

All employees new to Covenant Living must provide a criminal record check with vulnerable screening (CRC/VS) not older than 90 days ahead of your employment commencement date.
