Position Title: Life Long Care Program Support Worker

Organization: Kingston Native Centre and Language Nest (KNCLN)

Status: Full-Time (37.5 hours/week, primarily weekdays, with some flexibility for evenings and weekends as required)

Location: KNCLN Office and community-based sites, with travel as needed

Compensation: $21.00-$25.00/hour (based on experience)

The Kingston Native Centre and Language Nest is a Friendship Centre dedicated to supporting Indigenous families and communities through culturally safe programming, language revitalization, and wraparound services rooted in self-determination and community care.

About the Role

The Life Long Care Program Support Worker provides hands-on, person-centered support to community members with a focus on dignity, safety, and wholistic care, and supports the wellness and quality of life of Indigenous community members who are elderly, chronically ill, frail, and/or living with cognitive or physical disabilities.

In alignment with KNCLN's commitment to culturally grounded, wholistic, and trauma-informed care, this role coordinates health and wellness initiatives for clients and their caregivers, ensuring access to supportive services, cultural activities, and resources that promote dignity, independence, resilience, and community connection.

What You'll Do

1. Service Delivery

  • Provide culturally safe peer support, coaching, and supportive conversations for individuals and families

2. Program Development & Wellness Supports

  • Develop and deliver culturally relevant mental health and wellness programming
  • Apply evidence-informed peer support practices (e.g., motivational interviewing, strengths-based approaches)
  • Organize community workshops and cultural events in collaboration with Elders and Knowledge Keepers
  • Adapt supports to address the needs of individuals with complex needs, including concurrent issues

3. Case Navigation & Referral

  • Assess participant needs and provide navigation to internal and external supports
  • Advocate for access to housing, medical, social, and harm reduction services
  • Maintain up-to-date knowledge of local resources and referral pathways
  • Participate in case planning with service partners as required

4. Compliance & Reporting

  • Maintain accurate participant files, case notes, and program records
  • Complete all required reporting, including Serious Occurrence Reports (SOR/ESOR)
  • Ensure compliance with KNCLN policies and legal standards (OHSA, PHIPA, AODA, Violence & Harassment)

5. Collaboration & Partnerships

  • Work closely with the FWMH Manager and KNCLN staff teams
  • Engage Elders, Knowledge Keepers, and cultural advisors in program delivery
  • Build partnerships with health providers, harm reduction services, and housing agencies
  • Represent KNCLN at community events, coalitions, and interagency meetings

6. Other Duties as Assigned

  • Support cross-program initiatives and KNCLN community events
  • Facilitate group activities such as sharing circles, wellness sessions, and land-based healing
  • Support individuals in developing safety plans and coping strategies
  • Incorporate cultural teachings, ceremony, and traditional medicines into supports

  • Organized, adaptable, and dependable in dynamic settings
  • Ability to maintain healthy boundaries and model community values
  • Knowledge of relevant legislation including the Health Information Protection Act
  • Familiarity and commitment to confidentiality

What You Bring

  • Post-secondary education in Indigenous Studies, Social Work, Community Services, or a related field; or equivalent lived and community-based experience
  • Experience working with Elders, adults, or families in a community or wellness setting
  • Knowledge of Indigenous health and wellness issues and supports
  • Satisfactory Vulnerable Sector Check
  • Access to reliable transportation
  • Up to date CPR and First Aid certification
  • Valid driver's license and ability to drive independently

Work Expectations

Hours: Full-time hours Occasional evening and weekend work required.

Environment: Based at KNCLN with community-based programming and home visits as required

Physical: Light lifting, event setup, travel between sites

Emotional: Ability to support participants with complex needs while maintaining healthy boundaries

Health & Safety: Must comply with KNCLN health and safety protocols (OHSA, Violence & Harassment, WHMIS, PHIPA)
