At Rexdale Community Health Centre (RCHC), you'll work with an exceptional interprofessional team — physicians, nurses, social workers, dietitians, and mental health clinicians — delivering care in clinic, at partner sites, and on our Wheels to Wellness mobile health bus. We are recruiting a full-time permanent Nurse Practitioner to help expand access to high-quality, team-based primary care in one of Toronto's most diverse and vibrant communities.
RCHC is a multi-service organization providing primary health care, health promotion, and community support services. Our Interprofessional Primary Care Team (IPCT) works collaboratively to improve health outcomes and address the social determinants of health. As a Nurse Practitioner at RCHC, you will play a central role in providing comprehensive, patient-centered care to individuals and families, with a focus on those facing barriers to health care access.

Your Role
- Provide comprehensive primary care to clients of all ages in clinic, at community partner sites, and on the Wheels to Wellness mobile health bus
- Conduct physical examinations, order and interpret diagnostic tests, prescribe medications, and develop evidence-based treatment plans
- Deliver prenatal and postnatal care, including breastfeeding support and newborn assessments
- Support clients in managing chronic diseases through education, counselling, and care planning
- Lead and participate in health promotion, screening, and disease prevention initiatives
- Collaborate closely with physicians, nurses, social workers, and other health professionals to provide coordinated, integrated care
- Contribute to quality improvement, program evaluation, and innovative approaches to service delivery
What We're Looking For
- Bachelor of Nursing Degree + Extended Class Certificate of Competence (Primary Health Care NP)
- Current registration with the College of Nurses of Ontario and professional liability insurance required
- Affiliation with the Nurse Practitioners' Association of Ontario (NPAO) preferred
- NP experience in community-based primary care is an asset
- Strong commitment to culturally competent, equity-driven care
- Ability to work effectively both independently and in a collaborative team environment
- Familiarity with EMR systems (e.g., Telus' PS Suite) and strong computer literacy
- Strong commitment to equity-driven, community-based care
- Team-oriented, flexible, and ready to work in diverse settings
- Knowledge of the Rexdale Community is an asset
- Fluency in French, Spanish and/or Arabic is considered an asset

Location: We have 3 sites in North Etobicoke: 8 Taber road, 222 Dixon and 21 Panorama, plus some travel within North Etobicoke on our mobile health bus.
Salary: $108,091 - $130,231 (based on experience) plus HOOPP & a very generous Benefit and vacation package
Hours: 35 hours per week Monday to Friday 9am-5pm with the occasional evenings to 8pm and Saturdays
