Dr. Faiza Shafaqat Medicine Professional Corporation provides comprehensive primary care services within Stockyard Medical Family Practice & Walk-In Clinic, a multidisciplinary clinical environment serving a diverse patient population in West Toronto. Our practice prioritizes collaboration, evidence-based care, and a supportive, respectful work environment.

We are looking for a highly skilled, compassionate, and self-directed Nurse Practitioner to join our growing practice on a long-term basis.

Position Summary

The Nurse Practitioner will work collaboratively with Dr. Faiza Shafaqat to provide high-quality primary care to rostered patients. This position is ideal for an NP seeking stable long-term employment with consistent hours, autonomy, and ongoing professional growth.

You will manage your own patient schedule, perform assessments, order investigations, diagnose, prescribe, and provide health counselling according to your full scope of practice.

The Nurse Practitioner will provide clinical care both in person and virtually, with the majority of work being in person to support hands-on assessments, physical examinations, and continuity of care.

Responsibilities:

Clinical Care

• Perform comprehensive and focused assessments for patients across the lifespan
• Diagnose and manage common acute and chronic conditions
• Prescribe medications within NP scope
• Order laboratory tests, imaging, and consults
• Provide health promotion, chronic disease management, and preventive care
• Manage same-day/urgent care presentations where appropriate
• Develop care plans and follow-up strategies

Collaboration

• Collaborate with the supervising physician and clinic team
• Maintain accurate and timely EMR documentation (Avaros EMR)

Workflow & Administration

• Maintain accurate, timely clinical documentation in Avaros EMR
• Review labs, diagnostic imaging, and consult notes
• Complete clinical forms when needed
• Assist with preventive care reminders and quality improvement initiatives

Qualifications

Required:

• Primary Health Care Nurse Practitioner (NP-PHC) registered with the College of Nurses of Ontario (CNO)
• Active CNPS liability protection
• CPR certification
• Excellent clinical judgment and communication skills

Preferred:

• Experience in family medicine or community primary care
• Experience with Avaros EMR (training provided)
• Comfort working independently in a structured primary care setting

Hours & Schedule

• 8–16 hours per week
• 1–2 days per week (flexible options)

Job Types: Part-time, Permanent

Pay: $65.00-$70.00 per hour
