Position Description
The Bundled Care Navigator will provide patient centered system navigation, intensive case management and coordination of services to hip & knee replacement surgery patients within Trillium Health Partners and with partner organizations as part of the Bundled Care Program. Working collaboratively with an inter-professional team, both internal and external to THP, that include family members and support networks, the Care Navigator will identify and facilitate access to the right services from the right provider at the right time.
The Care Navigator is a key resource and link for home care and hospital staff in the care of hip & knee replacement surgery patients. The Care Navigator will coordinate seamless care transitions that link services and resources to enhance post-operative self-management and quality of care. The Care Navigator will act as a liaison between patients and other care providers.
The Care Navigator will be responsible for ensuring cost effective patient care by utilizing resources efficiently and collecting necessary data on client care to support the evaluation of outcomes of integrated care delivery. This program leverages technology to support patient care and seamless transitions from hospital to community and ongoing community management.
Position Requirements
- Degree in health profession of either Nursing, Physiotherapy or Occupational Therapy required.
- Current membership in good standing with a regulatory health professional body in Ontario.
- Three (3) years of recent related experience in a community health setting and experience in working directly with joint replacement surgery patients.
- Comprehensive knowledge of community resources and health care delivery systems in acute and community settings.
- Demonstrated ability to respond to patients with flexibility and adaptability.
- Experience in performing case management functions is required.
- Demonstrated expertise and competence in symptom management for joint replacement surgery patients and other areas of patient need, such as chronic disease management, complex family systems and the frail elderly.
- Excellent interpersonal, communication, organizational and decision-making skills required.
- Consistently demonstrated strong leadership skills, including problem solving, critical thinking, conflict resolution and negotiation skills.
- Demonstrated ability to work independently and collaboratively with an interdisciplinary team, family members, support networks, and a variety of formal and informal service providers.
- Excellent time management and stress management skills required.
- Fluency in English (verbal and written).
- Computer literacy and basic proficiency are required.
- A positive attendance record is required.
