Detailed Overview

In accordance with Fraser Health's Mission, Vision and Values and established standards and policies, works in collaboration with the multidisciplinary team to provide support functions such as client admission, intake, and discharge duties, orienting clients to services, monitoring client's physical and psychological status and assisting in the primary care aspect of the client's overall medical health. Initiates and maintains client charts and other necessary documentation. Provides information to clients and their families through group and/or individual discussions, supportive counselling, demonstrations, modeling and behaviour management regarding lifestyle, substance use issues, maintaining optimum health, and community services and resources. Completes referrals as appropriate. Participates in case planning and assists in the delivery of the clinical and primary care program. Facilitates clients' physical and recreational needs by leading exercise sessions and escorting and clients to off-site recreational activities. Assesses clients during intoxication, withdrawal and stabilization, and provides crisis intervention.
Responsibilities


  1. Performs routine care according to established standards and unit policies by performing duties such as assessing clients' physical and psychological status during intoxication, and withdrawal and stabilization; monitors and documents changes; takes and records vital signs such as blood pressure, pulse and respiration; reports changes in status to designated nursing staff as required.
  2. Maintains related records and documentation including medication records and client files by performing duties such as completing client documentation and updating records and files with changes as appropriate.
  3. Provides supportive counselling and guidance to clients and families. Interviews clients to identify needs and assists clients to determine appropriate services. Makes required referral to primary care providers, clinical counsellors and other professionals as needed
  4. Participates in case planning by performing duties such as interviewing clients to identify problems and needs, obtaining required information from a variety of sources such as families and care providers, providing input to members of the interdisciplinary team, and making recommendations on the development, implementation and evaluation of plans related to adolescents and/or adults.
  5. Performs client admission and discharge duties by performing duties such as meeting with clients to gather personal information including history and length of abuse. Develops discharge summaries, meets with client to ensure treatment plans are in place prior to client being discharged, and collaborates with Nurse and discharges clients as appropriate.
  6. Performs necessary follow up actions including scheduling client appointment, completing appropriate requisitions, liaising with laboratory and other agencies/departments, and flagging charts for the attention of the multidisciplinary team staff.
  7. Participates in client care plans by performing duties such as providing input to members of the interdisciplinary team and making recommendations on the development, implementation and evaluation of plans
  8. Participates in the coordination of emergencies under the direction of the Nurse including overdose, heart attack, assaultive behaviour and seizures by performing duties such as responding in accordance with established policies and procedures, providing crisis intervention, initiating appropriate responses including CPR and notifying ambulance and/or police as required.
  9. Provides personal care to clients in maintaining personal hygiene by performing duties such as assisting with bathing, dressing and toilet needs as required.
  10. Identifies available social, housing, economic, recreational and education services and resources in the community that will meet client's needs. Refers to and liaises with resources that will assist clients to meet their goals and improve their quality of life such as shelters, housing workers, dental clinics, outreach workers, home health, mental health, and integrated care management meetings.
  11. Participates in the therapeutic program by attending team conferences, and facilitating clients' physical, recreational, educational, and employment needs by performing duties such as leading discussion groups on topics such as lifestyle, basic health, nutrition, substance use, coping skills and behaviour management, leading exercise sessions, arranging for clients to participate in related programs/services, assisting clients to attend appointments, and implementing programs, escorting clients to off-site recreational and related activities.
  12. Works collaboratively with the clients using a strengths-based model, motivational interviewing and engagement techniques.
  13. Performs administrative support duties such as chart preparation and management, providing information regarding services and procedures to the general public, taking referrals, and referring non-routine inquiries. Gathers, compiles and inputs client information as required utilizing appropriate software applications such as word processing and databases. Prepares and maintains various informational/resource packages for clients.
  14. Participates in the orientation and education of new staff.
  15. Performs security duties such as maintaining unit security through routine search of client belongings and patrolling the premises and grounds as directed.
  16. Performs support duties in a clinical setting such as setting up and dismantling exam rooms; obtaining urine specimens for routine drug screening following established procedures; arranging for blood tests and diagnostic testing as directed; booking appointments; making referrals to specialists; and managing patient scheduling; observes and supports individuals in appropriate medication management, in accordance with the plan of treatment/care and established policies and procedures. 
  17. Plans and facilitates group support programs by determining content and structure in consultation and collaboration with Program staff and clients as appropriate, to encourage skill development in areas such as self-esteem, coping skills, motivation, the formation of mutual support networks, relapse prevention, and addiction services. Reports difficulties to Daytox Program staff according to procedures. Provides related feedback and follows up with clients as required.
  18. Performs other related duties as assigned.

Qualifications

Education and Experience

Completion of Substance Use Certificate plus one year's recent related experience working with clients with substance use issues, or an equivalent combination of education, training and experience. Successful completion of St. John's Ambulance Safety Orientated First Aide Certificate and current Cardiopulmonary Resuscitation Certificate (CPR) Health care level or equivalent.

Skills and Abilities
  • Ability to communicate effectively in English, both verbally and in writing, including empathetic and respectful communication appropriate to the situation.
  • Good interpersonal skills, including the ability to get along well with clients, families, and members of the interdisciplinary team, and including the ability to effectively network with other services and agencies.
  • Ability to perform word processing, spreadsheet and database tasks using appropriate software applications.
  • Ability to flexibly manage changing work load needs, setting effective priorities to manage demanding work load.
  • Ability to work independently and as part of a multidisciplinary team.
  • Ability to maintain confidentiality in accordance with applicable policies and codes of ethics. 
  • Familiarity with medical office procedures.
  • Demonstrated understanding of chemically dependent persons and their rehabilitation.
  • Ability to provide basic counselling support to clients and their families.
  • Ability to lead education and discussion groups.
  • Ability to respond appropriately to emergency situations.
  • Basic knowledge of substance use treatment approaches.
  • Knowledge of community resources and methods of accessing them.
  • Physical ability to carry out the duties of the position.
  • Ability to operate related equipment.
