Four Counties Addiction Services Team (Fourcast) is a community-based addiction treatment service provider that strives to provide the highest quality of evidence-based addiction treatment and case management services. Fourcast offers services across 5 sites in the region. The community treatment program staff work in-office and community settings and are often co-located and integrated with other organizations to reduce barriers for those accessing services.

About The Position...

This role is based in the Peterborough area and is a full time permanent position.

Fourcast's HART Hub Transitional Housing Program supports individuals exiting Wolf Street Modular Housing Program into Transitional Housing Program to supports service users on their path to independent housing. The program works from a Housing First approach and uses an evidence-based case management model that supports housing stability and recovery.

The Housing Intensive Case Manager (HICM) provides screening, assessment, treatment planning and interventions using addiction and housing evidence-based tools. Support is provided through assertive community engagement that includes active collaboration with HART Hub services, health, social services, landlords and housing partners.

What We're Looking For:

Education:

  • Bachelor of Social Work, Psychology, or related degree, with a postgraduate certificate in Addictions and Mental Health, or an equivalent combination of work experience and education.

Experience:

  • Minimum 3-5 years' experience working in a related position preferred, ideally in a community-based setting.
  • Advanced skills in addictions interventions including counselling/therapy, crisis intervention, advocacy, mediation, and conflict resolution.
  • Demonstrated experience working with people with serious mental illness and complex personal needs.
  • Experience using evidence-based screening and assessment tools.

Certificate, Licenses and Records

  • Valid Ontario Class G driver's license.
  • Valid First Aid/CPR.
  • Non-Violent Crisis Intervention (NVCI) certificate.
  • Certification in GAIN assessment would be an asset.
  • Applied Suicide Intervention Skills Training (ASIST) an asset.
  • Satisfactory Vulnerable Sector Check is required.
