Home and Community Care Coordinator -Vacant Position
Full-Time (37.5 hours per week) – Permanent
Salary Range: $57,795 – $71,081 annually
Location: Serving Atikameksheng Anishnawbek, Sagamok Anishnawbek, Serpent River First Nation and Mississaugi First Nation communities.

About Maamwesying North Shore Community Health Services
Maamwesying North Shore Community Health Services provides a full and coordinated continuum of care to the seven member First Nation communities of the Mamaweswen, North Shore Tribal Council; to urban Indigenous individuals in Sault Ste. Marie through the Indigenous Friendship Centre; and to four additional First Nation communities in the Wawa and Chapleau area.

In partnership with our 11 First Nation communities, we offer a wide range of services including primary care, traditional healing, mental wellness and addictions, home and community support, and health promotion.
Position Overview
The Home and Community Care Coordinator provides case management and coordination for clients with complex home care needs, with a focus on enabling Elders and seniors to live safely and independently in their own homes for as long as possible.
Working as part of an interprofessional team, the Coordinator ensures that clients receive the right mix of services and supports — medical, social, and cultural — to maintain quality of life, dignity, and connection to community.
Key Responsibilities
- Client Assessment: Assess client needs, eligibility, and risk factors, with a focus on supporting older adults and those with complex health needs.
- Care Planning: Develop individualized, holistic care plans that integrate clinical care, community supports, and traditional wellness practices.
- Case Management: Provide ongoing case management and coordination for clients, ensuring services are timely, effective, and culturally appropriate.
- Collaboration: Work closely with health professionals, traditional healers, hospitals, and community agencies to support smooth transitions and continuity of care.
- Client Support and Advocacy: Advocate for clients and families to access home care services, equipment, and other essential supports.
- Monitoring and Evaluation: Regularly review care plans, monitor progress, and adjust services as client needs change.
- Program and Community Support: Participate in health promotion and wellness initiatives that support seniors' independence and wellbeing.
Qualifications
- Post-secondary education in Health Sciences, Nursing, Social Work, Indigenous Studies, Gerontology, or a related field.
- Experience in home and community care coordination, case management, or health service navigation.
- Strong understanding of Indigenous cultures, values, and approaches to health and healing.
- Excellent communication, problem-solving, and organizational skills.
- Ability to work independently and collaboratively in a team environment.
- Valid Ontario driver's license and reliable vehicle for travel within the region.
Assets
- Experience working in or with Indigenous communities.
- Familiarity with local health and social resources in First Nation and surrounding areas.
- Ability to speak an Indigenous language is an asset.
Given the traditional practices of Indigenous people, from time to time exposure to smoke from the burning of sacred medicines such as tobacco, sweet grass, sage or cedar may occur.
