Our Community Support Services (CSS) team provides essential services that enable our clients to thrive. Whether their disability is physical, the result of brain injury, or aging, our more than 2,000 team members assist clients in their homes and across communities, to live as independently as possible.
The opportunity
We are looking to fill one position for a full-time Rehabilitation Service Coordinator in Edmonton, Alberta location.
Hours: 35 hours per week
Starting salary: $28.43 per hour
Location: Edmonton, Alberta
Responsibilities
- Plans and implements programs designed to enhance efficiency and safe delivery of Community Support Services to clients with various disabilities.
- Effectively manages and approves program resources, human and financial needs to meet program goals and objectives
- Recruits, employs, trains, supervises and evaluates support service staff, within a pre-determined budget in accordance with the approved program plans.
- To coordinate the assessment development, implementation or care plans.
- Monitors and evaluates operational effectiveness, and effects changes required for improvement.
- Participates as a member of CSS administration team in planning, problem resolution, quality improvement and review of service delivery performance.
- Act as a resource to staff and families in the provision of long-term support and service coordination for complex to enable adults with disabilities to remain in community.
- Act as a staff resource to support activities of Community Support Supervisor/Program Manager
- Valid drives license and access to vehicle
- Ability to travel regularly between MODC programs and the community of Edmonton, Alberta
- Compensated On-call on a rotational basis
Qualifications and experience
- Bachelor's degree/diploma in Human Services program or equivalent preferred
- 2 – 5 years' experience in the Social Services/Health Sector, coordinating services for individuals with a disabilities.
- Knowledge of cognitive impairment and mental health diagnosis
- Community based experience working with individuals with cognitive impairments, physical disabilities or special needs
- Strong written and verbal communication skills.
- Highly organized and excellent time management skills
- Demonstrated ability to supervise and train employees to include organizing, prioritizing and scheduling work assignments
- Problem solving and conflict resolution skills
- Knowledge of human relations
- Knowledge of behaviour management principles
- Knowledge of Health & Safety legislation
- General understanding of computer hardware and software
- Strong ability to assess and determine effective compensatory strategies to support maximum community participation/inclusion.
