Detailed Overview
As part of the -interprofessional and team-based care model, the Child and Youth Clinical Counsellor will provide therapeutic counselling services for children, youth, and young adults (up to 24 years old) and families experiencing mild to moderate mental health and substance use challenges. This role might include outreach work with local and neighbourhood-based resources such as; community hubs, centres, libraries, recreational centres, school district counsellors, and other youth friendly services. This role includes working closely with community and governmental organizations such as Ministry of Child and Family Development supporting children in care, Community Living BC, homeless outreach programs and services, Developmental Disability Mental Health Services, and other groups such as, outpatient clinics, family physicians, nurse practitioners, hospital and other community services staff for children and youth.
Responsibilities
- Provides short-term brief intervention and support to younger clients affected by mental illness and/or substance use, including children, youth, and young adults (up to age 24) in crisis, utilizing crisis intervention and short-term cognitive behavioural therapy, among other modalities based on treatment goals. Follows-up on, monitors, and evaluates treatment outcomes.
- Assesses client needs and available services and contributes to the shared care plan with clients' input. Facilitates clients' utilization of appropriate community services and rehabilitative programs through methods such as researching available services, liaising with the service and program providers, and providing advice and consultation to colleagues to ensure clients receive appropriate and timely service delivery and continuity of care.
- Screens referrals from the interprofessional team by gathering background information, determining service priorities, and providing timely response to referrals, through outreach/in-reach crisis intervention, short-term treatment stabilization, and/or referral to community-based supports.
- Collaborates with PCN interprofessional team, including family physicians and health care professionals, community partner and social service agency meetings as needed and as appropriate. Including system navigation for clients and families.
- Delivers workshops, psycho-education sessions, and ongoing training to clients, families, and community agencies as determined by the Manager, Clinical Operations & other PCN Management Team.
- Identifies gaps and problems with service and recommends solutions to assist in the development and maintenance of a coordinated community response to child, youth, and young adults (up to age 24) mental health.
- Maintains written reports and statistics and prepares written reports and clinical documentation according to established policies and procedures.
- Manages client feedback, develops treatment plans, and reviews accordingly, independently and in coordination with PCN Management Team.
- Makes referrals to community-based and specialized services such as Early Psychosis Intervention (EPI), START program, Foundry, Family Support Service, and other community resources for the purpose of providing continuity of care for children, youth, young adults (up to age 24) and their family support network.
- Cultivates a collegial atmosphere by actively participating and supporting team members with peer supervision and case consultation with the community Management Team, PCN team and Clinical Coordinator.
- Assists in the evaluation of the program and in monitoring quality improvement activities, standards of practice, policies, and procedures.
- Performs other related duties as assigned.
Qualifications
Education, Training & Experience
Master's in Counselling Psychology, Social Work or other related discipline from an accredited educational institution, plus two (2) years of recent related experience in working with families, children, and youth in a clinical counselling setting; or an equivalent combination of education, training and experience.
Eligible for registration with relevant professional association.
Skills & Abilities
- Comprehensive clinical counselling and therapy skills including crisis counselling.
- Demonstrated comprehensive knowledge of child and youth mental health psychiatric assessment skills, crisis intervention techniques, de-escalation techniques, brief/short-term therapy skills and interventions, general counselling theory/principles, community response procedures, community resource utilization and knowledge of psycho-pharmacology.
- Demonstrated comprehensive knowledge of substance use, relapse prevention and harm reduction.
- Demonstrated ability to deal with others effectively, including establishing and maintaining rapport with psychotic, crisis-prone, non-compliant people, and working collaboratively and co-operatively with other care providers to facilitate and enhance interprofessional, inter-departmental, and interagency communication and problem solving.
- Ability to communicate effectively both verbally and in writing.
- Demonstrated ability to network with other services and agencies.
- Ability to work on interprofessional teams effectively, and to plan, organize, and prioritize work.
- Ability to operate related equipment including applicable software applications.
- Physical ability to carry out the duties of the position.
