An exciting opportunity as a
Regular, Full-Time Social Worker
in the
Transition Related Surgery (TRS)/Gender Affirming Surgery Program.
Summary Of Duties, But Not Limited To
Patient Assessment
- Conduct comprehensive mental health assessments for patients undergoing gender-affirming surgery, with a focus on individuals with complex psychosocial or medical needs using a trauma-informed, person-centered, gender-affirming approach.
- Formulate assessment findings and determine the suitability for program resources
- Consult with team members and participate in team discussion regarding assessment and treatment, when required.
- Complete TRS surgical readiness assessments and provide guidance regarding timing and preparation for surgery.
- Liaise with community providers and social service agencies, when required.
Care Planning
- Develop an intervention plan in collaboration with patients, families, significant others and the interdisciplinary team, based on identified needs.
- Document intervention plan according to specific hospital and professional standards.
- Provide case management and waitlist management support in collaboration with community and referring partners.
- Coordinate care for patients with complex needs, including facilitating access to aftercare, mental health supports, community resources, and other social determinants of health.
- Support patients in preparing for surgery, including providing education and anticipatory guidance
Interventions
- Provide individual and group therapy/treatment using the principles of trauma-informed, person-centered, gender-affirming approach, and draw upon recognized psychotherapy models
- Consult and collaborate with multidisciplinary team members, as required.
- Provide short-term, problem-focused psychosocial support.
- Deliver crisis intervention, emotional support, and post-surgical adjustment counseling as needed.
- Facilitate peer support programming, including psychoeducational groups for patients undergoing TRS procedures.
Team and Program Development
- Participate in program development, student education and research as required in a fully affiliated academic setting within the University of Toronto.
- Participate in personal professional development.
- Develop and/or facilitate group programming and peer support
- Coordinate the TRS Compassion Fund
- Organize and support Psychiatry Rounds and TRS Complex Care Rounds
- Contribute to the development of patient education materials and accessible resources for patients and their supports.
- Participate in research activities including recruitment, data collection and data organization and interpretation.
As a role model and champion you will work to identify and integrate safe, best practices into daily activities to foster the delivery of safe and exemplary care.

Qualifications
- Current registration in good standing with the Ontario College of Social Workers and Social Service Workers, required
- Masters Degree in Social Work required
- Minimum 5 years clinical mental health experience, preferably in a hospital setting.
- Demonstrated experience with a variety of psychotherapies including cognitive behavioral therapy, interpersonal therapy, psychodynamic, crisis intervention, and group therapy.
- Experience in supporting transgender and gender-diverse adults to access gender-affirming care is an asset.
- Demonstrated knowledge of gender-affirming care, trauma-informed practice, anti-oppressive practice, and community resources relevant to trans, non-binary and gender-diverse populations.
- Comfortable facilitating both psycho-educational and process groups is preferred.
- Research experience or interest in quality improvement initiatives is an asset.
- Ability to work independently and manage complex and high-acuity patients.
- Excellent time management skills and organization skills.
- Proficient in Microsoft Office.
- Experience with electronic medical records (ie EPIC) is an asset
- Exceptional written/verbal communication skills.
- Proven attendance and punctuality.
- Professional behaviour and communication that meets the standards of the professional regulatory college or association, as applicable, and the standards of Women's College Hospital
