Outreach Manager

We are seeking an individual who is passionate about promoting health and wellness equity in the Downtown Eastside (DTES) to join the Kilala Lelum Health Centre as our Outreach Manager. Kilala Lelum aims to provide access to primary care and Elder-led cultural care to new and existing members of the Health Centre through an outreach-based, culturally-safe, trauma-informed, member-centered, and harm reduction approach.

Job Summary:

The Outreach Manager will ensure the effective delivery of integrated, trauma-informed, and culturally safe care to our members in the DTES. This role is pivotal in fostering a supportive team environment, managing program operations, and strengthening partnerships with community agencies to advance our mission of improving healthcare access and cultural connections for our members.

Key Responsibilities

1. Team Leadership & Staff Management:

2. Recruit, train, supervise, and mentor a team of Outreach Workers across Primary Care, Relational Care, Social Navigation, Food Security, and Housing.

3. Develop staff schedules, assign outreach routes and Member appointments, and ensure adequate coverage.
4. Facilitate regular team meetings, Member conferences, and debriefing sessions.

5. Conduct performance evaluations and support ongoing professional development for team members.

6. Program Coordination & Development:

7. Oversee the day-to-day operations of the outreach program, ensuring alignment with the integrated health service model.

8. Develop, implement, and evaluate outreach strategies and protocols to enhance service effectiveness and member outcomes.
9. Coordinate with the Primary Care, Relational Care, Social Navigation, Food Security, and Housing teams to ensure a seamless continuum of care for members.

10. Manage the distribution and inventory of supplies, including harm reduction materials, medications, and food.

11. Community Partnership & Liaison:

12. Act as a liaison between Kilala Lelum and other community agencies, health services, and partners.

13. Build and maintain strong collaborative relationships to improve resource access and coordinate member care.

14. Represent Kilala Lelum at community tables and networking events.

15. Administrative & Operational Duties:

16. Ensure timely and accurate documentation standards are met by the team, including the use of the OSCAR EMR system.

17. Monitor and report on program metrics, outcomes, and key performance indicators.
18. Assist in the preparation of reports for funders and internal stakeholders.

19. Uphold and enforce policies related to confidentiality, safety, and risk management.

20. Clinical & Case Support:

21. Provide guidance and support to staff in complex situations, including crisis intervention and conflict resolution.

22. Assist the team with complex case planning and advocacy challenges.
23. Ensure all outreach practices adhere to the principles of trauma-informed care, harm reduction, and cultural safety.

Qualifications and Skills

Required:

• A degree in Social Work, Health Sciences, Public Administration, or a related field, plus several years of relevant experience; or an equivalent combination of education and extensive experience in a community health or social services setting.
• Minimum of 2 years of experience in a supervisory or team leadership role.
• Demonstrated knowledge and experience providing culturally safe and competent care, specifically pertaining to Indigenous people.
• Deep understanding and practical application of trauma-informed practice, harm reduction, and recovery-oriented care.
• Proven experience working with people with mental health and substance use disorders in the DTES or a similar context.
• Strong ability to deal effectively with conflict, manage aggressive behaviour, and support staff in crisis situations.
• Excellent interpersonal skills with the ability to establish and maintain satisfactory working relationships with the public, staff, and community partners.
• Superior verbal and written communication skills.
• Proficiency with basic computer software and electronic medical records (experience with OSCAR is a strong asset).
• Knowledge and experience working with Indigenous peoples and culture; completion of a cultural competency course or willingness to participate is required.
• Overdose response training or willingness to be trained immediately.

Preferred:

• Lived experience and/or deep connections to the Downtown Eastside community.
• Formal training in Indigenous Cultural Safety or related disciplines.

Working Conditions:

• Outreach duties may occur in the DTES in hotels, SROs, and private apartments following the safety guidelines outlined by the KL team. Main office location will be at 687 Powell Street.
• Physical ability to carry out the duties of the position including helping members who use mobility devices, lifting up to 15lbs, and walking up to 5-10km per day.
• A driver's license is required.

Supervision Provided:

This position is supervised by and will receive support from the KL Leadership Team.

Additional Information:

Ideal start date is November 2025.

Successful candidates will be required to complete a Criminal Record Check.

Hourly wage of $38.38/hour to start

Hours of work will be agreed upon to meet requirements of 0.8 FTE (32 hours/week).
