Job Description
- Provides support to parents of children aged birth to 5 in their home and in the community. The program focuses on attachment, parenting skills, health, and safety. The support worker provides developmental screeners and support to parents in the form of information and modelling to families regarding development, play activities, and community resources. The program can help support leisure opportunities and food security in the form of recreation passes, grocery gift cards, and bus passes (quantity of this support depends on funding). The support worker works as part of a multi-disciplinary team and may make referrals to other services and early intervention therapies if needed. Program is funded through MCFD.
Duties and Responsibilities
- Participate as a multi-disciplinary team using a family-centered model of service delivery.
- Meet and consult with families weekly to assess their needs and establish an effective service plan with the family.
- Share knowledge of community resources to meet the clients' needs.
- Write effective service plans with measurable goals and objectives with client and team.
- Provide service coordination for families. Agreed upon consultation at service coordinator review meetings, held quarterly. Coordination including being the family's main contact, facilitating service plan meetings and confirming service plans are complete and up to date.
- Client charting, documentation, and statistical information regarding clients for all report needs by the Centre and funding agencies.
- Be able to manage and organize workload and manage time, clients, prep, documentation, meetings, consultations, etc.
- Perform and be able to evaluate developmental screeners.
- Provide family support as needed to decrease barriers. May be required to transport clients.
- Attend meetings as requested.
- Involvement in monthly Good Food Box program; order for clients, sort and deliver.
- Facilitate Building Blocks Family Play Drop-In Group and perform duties related to facilitation of a group with children and families.
- Continuing competence will be demonstrated through ongoing professional development to assist members to maintain professional knowledge, abilities, and skills.
- Recognize own limitations and consult, refer or delegate when needed. Consult with manager when needed regarding program and client progress.
- Perform other duties and tasks as may be requested.
Qualifications and Skills
- Diploma in social work, child and youth care or equivalent degree and/or four years of experience in related field.
- Ability to communicate effectively with families, parents and children and function as part of a multidisciplinary team.
- Ability to promote healthy family functioning.
- Understanding of child development and developmental milestones of children ages 0-5.
- Ability to work independently and in a team environment.
- Knowledge of other community agencies and their resources pertinent to families attending.
- Knowledge of car seat installation and regulations.
- Demonstrate ability to adhere to and implement the KCDC's principles, policies and practices including understanding and maintaining confidentiality.
- Must be physically able to perform aspects of the position unless an accommodation agreement is in place.
Additional Requirements
- Available Monday to Friday and be able to work flexible hours as needed.
- Current drivers' licence, driver's abstract and personal vehicle for work use
- Completed criminal record check
