Job DetailsDescriptionPosition SummaryThe Ontario Shores Centre for Mental Health Sciences is seeking a Case Worker reporting to the Clinical Manager of the Women's acute stabilization Unit. The successful candidate will be a Case Worker with sound knowledge of clinical issues in the area of mental health and addiction.Responsibilities IncludeParticipating as a mental health professional on an interprofessional team, and liaison with community partnersReview incoming referrals from correctional facilities across Ontario to assess eligibility and suitability for the stabilization programCommunicate directly with jail staff, case managers, and institutional mental health clinicians to gather background information and clarify referral detailsParticipate in admission‑screening meetings with both the unit's treatment team and correctional partners to determine whether a client is an appropriate candidate.Conduct intake interviews with admitted clients to gather psychosocial history, risk factors, and support needs.Develop individualized support plans in collaboration with the interdisciplinary team, based on non‑clinical assessment and client goals.Participate in case conferences, interdisciplinary meetings, and daily team discussions to support integrated care.Advocate for client needs within the team and with external partners, including correctional staff and community agencies.Coordinate discharge planning, including referrals to community supports, shelters, treatment programs, and correctional contacts when returning to custody.Maintain accurate, timely, and confidential documentation, including case notes, incident reports, and discharge summaries.Uphold professional boundaries, confidentiality standards, and organizational policies at all times.Provide client centered assessment, treatment and support to referred clientsReviewing referrals and assessments in consultation with the team to determine treatment optionsParticipating in the development of treatment / rehabilitation goals and plans in collaboration with clients, their families, their support network and the interprofessional teamProvide resources and services to client's and families to support reintegrationTo participate in research and program evaluation activitiesQualifications Required And PreferredQualifications Required and Preferred;Registration as a Healthcare Provider, Undergraduate Degree, or possession of a current certificate of competence as a Registered Practical Nurse if applicable.Minimum 3–5 years' experience in mental health and/or addictions sector, including intake, navigation, or case managementOther Requirements: CPR-HCP/BLS1 Vacancy AvailableLink to OPSEU Wage Table -This is part of the OPSEU bargaining unit.Job Posting Closing: January 27, 2026 at midnight.
