Overview
We are seeking a highly skilled Nurse Practitioner (NP) specializing in Cardiovascular Medicine and Clinical Lipidology to join our multidisciplinary outpatient team.
This role offers an opportunity to provide expert patient care in a dynamic clinical setting, utilizing advanced skills in cardiovascular assessment, lipid management, and comprehensive patient education.
The ideal candidate will have extensive experience working with diverse patient populations, including those with complex cardiac conditions, and possess proficiency with EMR system like ACCURO.
This position is integral to delivering high-quality outpatient care, with a focus on improving cardiovascular health outcomes through evidence-based practices and innovative treatment modalities.
Position Summary
We are seeking a Nurse Practitioner to perform patient assessments, manage chronic cardiometabolic conditions and lipids disorders, interpret diagnostic testing, and work collaboratively with our specialist in outpatient setting.
Responsibilities
- Conduct thorough cardiovascular assessments, including physical examinations, vital signs, and physiological evaluations.
- Manage complex lipid disorders through personalized treatment plans, including medication management and lifestyle counseling.
- Perform diagnostic evaluations utilizing laboratory testing, medical imaging, sonography, to inform clinical decisions.
- Manage cardiovascular diseases including coronary artery disease, heart failure, valvular diseases, atrial fibrillation and arrhythmias, dyslipidemia, hypertension, metabolic syndrome, obesity.
- Document all patient encounters accurately within EMR systems like ACCURO, adhering to HIPAA regulations.
- Collaborate with physicians, specialists and ancillary staff to coordinate comprehensive care plans.
- Educate patients on cardiovascular health, medication administration (including injections), nutrition, and health coaching strategies.
- Participate in clinical research initiatives related to lipidology or cardiovascular medicine; contribute to quality improvement projects aligned with NCQA standards.
- Engage in case management, utilization review, and telehealth consultations to optimize patient outcomes across outpatient settings.
Qualifications
- Current Certification of Registration with the College of Nurses of Ontario (CNO) as a Registered Nurse, Extended Class with Nurse Practitioner (Adult).
- Demonstrated experience working with EMR/EHR systems such as Epic, Cerner, ACCURO, Athenahealth, or eClinicalWorks.

Compensation & Benefits
- Salary: $62–$72/hour (based on experience).
