Overview
We are seeking compassionate and dedicated Support Worker to provide high-quality care and assistance to individuals in various care settings. This position is primarily for weekends and evenings. This role involves supporting residents with daily activities, health monitoring, and emotional well-being, ensuring a safe and nurturing environment. The ideal candidate will have experience in home and community care, assisted living, or long-term care, along with a strong commitment to patient-centered support. Prior training in first aid, CPR, and HIPAA compliance is highly desirable. This position offers an opportunity to make a meaningful difference in the lives of those requiring assistance with daily living and healthcare needs.
Responsibilities
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by taking vital signs such as blood pressure, pulse, and temperature
- Administer medications and assist with medication management according to care plans
- Support residents during meal preparation and feeding, ensuring nutritional needs are met
- Provide companionship and emotional support to promote mental well-being
- Maintain a clean and safe environment through regular cleaning and sanitation practices
- Observe and report any changes in residents' physical or mental health to appropriate healthcare professionals
- Assist with transfers using equipment such as Hoyer lifts or resident lifts for heavy lifting needs
- Follow all HIPAA regulations to ensure confidentiality of patient information
- Support residents with specialized needs such as dementia care, Alzheimer's care, autism support, or hospice care as required
- Collaborate with healthcare teams to develop and implement individualized care plans
- Document patient observations accurately and maintain detailed records in compliance with healthcare standards
Qualifications
- Prior experience in home & community care, assisted living, nursing homes, or long-term care preferred
- Certifications such as First Aid, CPR, and HIPAA compliance are highly desirable
- Experience working with individuals with developmental disabilities or neurodegenerative conditions such as dementia or Alzheimer's is a plus
- Ability to perform heavy lifting and assist with mobility using appropriate equipment (e.g., Hoyer lifts)
- Strong communication skills and patience when working with diverse populations including seniors and individuals with disabilities
- Knowledge of medication administration protocols and vital signs monitoring
- Compassionate attitude with a genuine desire to improve the quality of life for residents
- Ability to work independently as well as part of a multidisciplinary team in home or community settings
