SALARY RANGE: CA$115,000 - CA$160,000 depending on experience.
Job Summary
We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will possess a strong background in patient care, with experience in various settings including pediatrics, senior care, and acute care. This role requires a commitment to providing high-quality healthcare services, managing patient cases effectively, and collaborating with other healthcare professionals to ensure optimal patient outcomes.
Responsibilities
- Conduct comprehensive assessments of patients, including physical examinations and medical histories.
- Develop and implement individualized care plans based on patient needs and preferences.
- Administer medications and treatments as prescribed, ensuring adherence to medical protocols.
- Provide education and support to patients and their families regarding health management and disease prevention.
- Collaborate with multidisciplinary teams to coordinate patient care across various settings such as hospitals, nursing homes, assisted living facilities, and home care environments.
- Utilize electronic health record systems (Epic or Cerner) for documentation and management of patient information.
- Perform diagnostic tests and interpret results, including laboratory work and phlebotomy when necessary.
- Manage chronic conditions such as dementia care, pain management, and other acute health issues.
Qualifications
- Master's degree in Nursing or a related field from an accredited program.
- Current Nurse Practitioner certification with a valid state license to practice.
- Experience working with children, seniors, or in acute care settings is highly desirable.
- Proficiency in medical terminology and case management practices.
- Strong clinical skills in patient assessment, medication administration, and treatment planning.
- Familiarity with electronic health record systems (preferably Epic or Cerner).
- Excellent communication skills with the ability to build rapport with patients from diverse backgrounds.
