KEY FUNCTION:
Provide best practice assessment, treatment, navigation and support to individuals residing within a Home First transitional residential community program, with a primary diagnosis of substance use disorder.
REPORTING:
Under the general direction of the Clinical Manager, Lotus Program.
DUTIES:
1. Work in collaboration with an interdisciplinary team to enhance client's skills and abilities to live independently following discharge from a transitional housing program.
2. Liaise with applicable psychiatric and medical units to obtain information regarding care planning and ensure the continuity of care from an inpatient to outpatient setting.
3. Liaise with Addictions Medicine Consult Team (AMCT), Rapid Access Addictions Medicine (RAAM) clinic, Outpatient Addictions and Gambling Service (OAGS) and any relevant HSN or community agencies to support the continuation of care or service needs.
4. Liaise with City of Greater Sudbury client navigators and other community health and social services to provide wrap around services to clients residing in the program.
5. Complete the development and implementation of treatment plans with residents.
6. Complete assessments for treatment planning purposes with clients and caregivers.
7. Document assessments, intervention plans, progress notes and outcomes in a timely manner as per program policy standards.
8. Assist in and act as a resource for client and caregiver education.
9. Consult and liaise with appropriate resources to ensure quality care and maintain patient safety.
10. Provide guidance, training and orientation for new staff and students.
11. Establish and maintain pathways to refer clients to on-going services post discharge from the program.
12. Facilitate a variety of education sessions and group treatment/counselling sessions.
13. Utilize a variety of strength based, therapeutic techniques including but not limited to harm reduction approaches and motivational counselling.
14. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
15. Participate in the accreditation process by engaging in best practices, audits, assessments and improvement initiatives.
16. Determine and align improvement projects with HSN's Strategic Plan; monitor and adjust to achieve goal outcomes.
17. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
18. Educate and promote health, safety and wellness in the workplace.
19. Represent the department or program on various committees and in meetings as required.
20. Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a one (1) year diploma in Health or Social Services field, from an accredited college.
2. Certificate in Basic Cardiac Life Support (BCLS) Automated External Defibrillator (AED) First Aid, and Non-Violent Crisis Intervention (NVCI) are preferred.
3. Current Certification or eligible to obtain the International Certified Alcohol and Drug Counsellor (ICADC) is preferred.
4. Ministry of Labour "Worker Health and Safety Awareness in 4 Steps" training certificate is required.
EXPERIENCE:
1. Minimum of two (2) years' full-time equivalent experience working in a Mental Health and Addictions.
2. Experience in community-based, out-patient programs is required.
3. Experience in program development and group facilitation is preferred.
KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated knowledge of addictions related care (i.e. withdrawal symptoms, withdrawal management, and medication treatment available).
2. Demonstrated knowledge of evidence based practices of recovery principles and stages of change.
3. Demonstrated ability to complete addictions assessments, treatment planning, and referrals.
4. Demonstrated ability to practice in the areas of clinical counseling with adults and families with strong client and family service orientation.
5. Experience providing both individual and group interventions.
6. Demonstrated excellent judgment, critical thinking and proven analytical skills.
7. Demonstrated knowledge of how to apply harm reduction practices.
8. Demonstrated knowledge of culturally safe practices.
9. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
10. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
11. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
12. Demonstrated compliance with HSN's privacy policy.
13. Demonstrated superior interpersonal and communication skills, both written and verbal.
14. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
15. Demonstrated discretion and maturity when handling confidential information.
16. Demonstrated commitment to the safety of co-workers and patients.
