About This Opportunity
The
In-Home Restorative Care Program
will provide person centered care at home for individuals who have recently experienced a loss of functional ability because of a decline in health status or after a medical event. The goal will be to increase strength, mobility, and functional ability to allow clients to return to an established baseline that enabled them to remain at home for as long as possible. While admitted to the In-Home Restorative Care Program individuals will receive medical and nursing oversight, active rehabilitation programming, access to social work and access to personal care support depending on the individuals assessed need. Through a collaborative process including both AHS and CBI Home Health, personalized care plans will be established and will include goal setting for a short term rehabilitation and other restorative care services, in alignment with AHS Rehabilitation Model of Care.
Restorative Care Is Defined As
A type of rehabilitation that enables clients to regain or retain their independence following the debilitating effects of acute illness, injury, or prolonged hospitalization. It is a time limited, multi-disciplinary, goal oriented and client-centered approach. Restorative care helps clients, across the continuum of care, return to home and remain at home as long as possible. It mitigates the risk or need for higher levels of health care or premature admission to a Continuing Care living option. Restorative care helps medically stable clients learn or re-learn the necessary skills required to complete their daily living activities. Restorative care can also refer to a health service setting (see above description)
The services will be provided by an integrated team that includes a case manager, regulated health professionals such as OT, PT, SW, a Nurse Practitioner and rehab assistants with expertise in working with the frail elderly population and/ or adults living with complex chronic conditions.
Your Role
- Provide assessment and treatment to a wide variety of clients with varying needs, including those with chronic and/or mental health conditions
- Provide impartial home, job, and safety/attendant care assessments, functional ability evaluations, insurer examinations, ergonomic assessments, client education, and job coaching, as required
- Be responsible for managing and prioritizing clinical caseloads, and for the coordination and delivery of therapeutic services
- Participate in a collaborative care model providing holistic, comprehensive care to meet the needs of clients
- Promote positive relationships between clients, team members, funders and the community
What You Will Need To Be Successful
- You are registered as a member in good standing with the College of Occupational Therapists of Alberta
- Ability to travel within the Calgary area (valid driver's license and reliable car is required)
- Health Care Providers Level C CPR certification issued within the last 12 months prior to commencement of employment
- Your focus is to embrace person and family-centered care
- You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions
- You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
- You value the role a therapist plays in keeping clients at home when that is of personal importance
