Range of Pay: Group 12 - $34.10 to $43.02 per hour

Characteristic Duties: Under the general supervision of the Co-ordinator of Shelters and Homelessness.

  1. Meet with clients to determine their ongoing needs to link them to appropriate health and social services. Support them to moving to life stabilization.
  2. Perform case planning, monitor and support the client's progress in addressing barriers through various methods (e.g., one to one support), and motivate and facilitate participation in programs.
  3. Connect the client with appropriate housing supports or services based on their needs including supportive and transitional housing programs, addiction treatment programs, housing case management, and income support.
  4. Provide supports or services based on their needs including physical health care, mental health care, addiction services, and trauma services.
  5. Work collaboratively with other front-line agencies that typically provide service to the same population including emergency shelter staff, community outreach teams, police, housing case managers, withdrawal management services, and other social services.
  6. Follow up with clients who have been housed, to assist them to connect with necessary wrap around services to support them to stay housed. Including the collection and verification of information required to determine initial and ongoing community placements.
  7. Attend regularly at the Emergency Shelters, Samaritan Centre, and other community services to meet with clients and provide resources.
  8. Participate in the Rapid Mobilization Table or other community initiatives for the benefit of supporting clients.
  9. Maintain a strong working knowledge of community services and resources.
  10. Identify and report on gaps in services and supports for housing stability for persons who are experiencing persistent homelessness.
  11. Complete required documentation and forms, record relevant case information, and record statistics as required (i.e., HIFIS).
  12. Develop and maintain a thorough working knowledge of City of Greater Sudbury's (CGS's) Safety Manual and the applicable provincial legislation listed therein.
  13. Perform other related duties as required.

Qualifications:

  • Successful completion of a college diploma in Social Services or a related field from a recognized community college with Canadian accreditation.
  • Life skills training considered an asset.
  • Minimum of two (2) years work experience in Social Services addictions or mental health.
  • Experience addressing the needs of individuals and families experiencing homelessness or those at risk of homelessness including developing and implementing successful case management plans.
  • Experience in assisting persons living with addictions, mental health, psycho/social/physical, education, emotional, and/or environmental concerns in a social service setting.
  • Experience with conflict resolutions practices including "in the moment" problem solving and intervention.
  • Ability to enhance participant motivation in order to make and follow through on decisions related to housing stability and wellness.
  • Understanding of community agencies and a cross section of service providers.
  • Knowledge of services to support this population and an ability to provide participant advocacy and proactive participant planning with other system elements.
  • Competent interpersonal and communications skills and demonstrated ability to serve the public in a courteous and effective manner.
  • Proven ability to work with computer software in a Windows environment (e.g., file maintenance, word processing, spreadsheet applications, information input and retrieval, etc.).
  • Demonstrated report writing skills.
  • Excellent use of English; verbally and in writing.
  • French verbal and written skills an asset.
  • Provide, at own cost, a current Criminal Reference Check.
  • Satisfactory health, attendance, and former employment history.
  • Must be physically capable of operating a vehicle safely, possess a valid driver's licence, have an acceptable driving

record, and personal insurance coverage.

Competencies: Competency Library - Level 1 Proficiency

This job is also being posted as a development opportunity. Should there be no fully qualified candidate for this position, a candidate who can be expected to meet the required qualifications within a reasonable period of time may be considered for this position as a development opportunity.

Development opportunity range of pay: $31.79 to $39.50 per hour. The successful candidate will be paid at the reduced range until the minimum qualifications have been met.
