Clinical Practice Leader

The Clinical Practice Leader (CPL), Inpatient Diabetes works with the leadership team within the clinical area and is responsible for provision of visible clinical support, coaching and modeling to ensure that the care provided in the hospital setting meets current professional and health care standards. The incumbent will work in collaboration with the interprofessional team to facilitate optimal patient care through education, providing resources and protocol development in a way that is consistent with Mackenzie Health's vision, mission and strategic direction. They will serve as a bridge between the inpatient/acute care setting and the outpatient Diabetes Education Centre. The incumbent will support the new CVH Hospital while working collaboratively with the CPL Diabetes incumbent at the Richmond Hill site.

What must you have?

• Registered Nurse in good standing with the College of nurses of Ontario required.
• Baccalaureate degree in Nursing required.
• Currently certified with the Canadian Diabetes Educator Certification Board (CDE) required.
• Current Basic Cardiac Life Support required.
• Minimum of five (5) years of relevant diabetes experience in clinical practice.
• Minimum of two (2) years of progressive leadership experience.

• Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
• Ability to perform the essential duties of the job.
• Effectively uses empathy in interactions with others.

Preferably, your profile also includes:

• Master's degree preferred.
• Member of the Diabetes Canada preferred.
• Professional Association Membership preferred.
