Job Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively within a multidisciplinary team. The LPN will be responsible for administering medications, monitoring patient health, and supporting patients' physical and emotional needs to promote optimal health outcomes.

Responsibilities

  • Deliver comprehensive patient care in diverse settings such as acute care, senior care, hospice, pediatrics, and dementia care.
  • Administer medications and treatments accurately following physician orders and established protocols.
  • Monitor vital signs and observe patient conditions to identify any changes requiring medical attention.
  • Assist with procedures such as phlebotomy, laboratory tests, and pain management interventions.
  • Document patient information accurately using electronic health records systems like Epic and Cerner.
  • Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans.
  • Educate patients and their families about health conditions, medication management, and post-discharge care instructions.
  • Ensure compliance with all safety standards and infection control protocols to maintain a safe environment for patients and staff.
  • Support case management activities by coordinating services and resources tailored to patient needs.

Skills

  • Proven experience in nursing home, hospital, or home care settings with a focus on patient-centered care.
  • Strong knowledge of medical terminology, medication administration, and clinical procedures such as phlebotomy.
  • Familiarity with electronic health record systems including Epic and Cerner.
  • Excellent communication skills for effective interaction with patients of all ages—including children—and multidisciplinary teams.
  • Ability to handle complex cases involving dementia care, hospice support, pain management, or senior care.
  • Experience with laboratory procedures or laboratory data interpretation is a plus.
  • Capable of managing multiple priorities efficiently while maintaining attention to detail in fast-paced environments.
  • Compassionate attitude with a commitment to delivering respectful and empathetic patient care.

Job Type: Casual

Pay: $21.93-$26.00 per hour
