Position Description

The Hospital for Sick Children (SickKids) is a leading specialty pediatric hospital providing complex, innovative care and treatment to children, youth, and families. Serving a diversity of patients and families, SickKids social workers are members of the Brain and Mental Health Program and deployed across specialty divisions of the hospital providing a broad range of important clinical supports and interventions to ensure caregivers, families and patients receive wholistic, comprehensive care, treatment, and transition planning. Providing services in one of the most diverse cities in the country, SickKids social workers are committed to enhancing health equity, respectful service and inclusion of all populations served by the hospital.

Reporting to the Clinical Director of Social Work and the Senior Clinical Manager, Tele-Mental Health, the Social Worker will be a collaborative partner with the Nurse Practitioner providing Gender Care across the province as part of the Tele-Mental Health Service. The Tele-Mental Health Service ' is a consultation program that offers psychiatric, mental health, and gender care assessments for rural, remote and underserviced areas of the province through three pathways: clinical consultations, program consultations, and education. This position will provide social work services for the youth and caregivers that are referred for Gender Care. The Social Worker and NP will identify adolescent and family strengths, problem-solving abilities, interactions, attitudes, feelings, and cultural factors in relation to gender identity. The social worker will also work with local agencies to identify resources available to the youth and family through partnerships with community agencies.

The team will develop and recommend plans of intervention that address coping, relationships, and quality of life. This may be achieved through a variety of modalities including individual sessions, group facilitation, education, program consultations, and community outreach and advocacy.

Here's What You'll Get To Do:

  • Conduct virtual psycho/social consultations working in collaboration with other members of the team to develop and recommend intervention plans that address coping, adaptation, and quality of life. This is achieved through a variety of modalities including trauma-informed counselling, consultation, case management, crisis intervention, community outreach, and advocacy
  • Provide virtual brief intervention, supportive counselling for youth and caregivers referred to the TMHS
  • Assume responsibility for all aspects of practice including issues of productivity and utilization
  • Collaborate/communicate with community agencies.
  • Practice in a manner that is consistent with the philosophy of continuous quality improvement and evidence-based practice.
  • Collaborate with case managers and teams from across the province
  • Provide education to other members of the team in terms of social work interventions that benefit patients and families

Here's What You'll Need

  • A Master's degree in Social Work.
  • Registration with the Ontario College of Social Workers and Social Service Workers.
  • An awareness and appreciation of the challenges youth with diverse gender identities face
  • Preference will be given to candidates with knowledge and experience working with adolescents and with parents
  • Preference will be given to candidates with lived experiences, knowledge, and education of working with non-binary, gender-expansive, trans, gender diverse/fluid or non-conformity individuals
  • Preference will be given to individuals who have knowledge and experience working in northern/rural communities, as well as experience working with Indigenous communities and knowledge of intergenerational trauma and ongoing impacts of colonialism/colonization
  • Well-developed assessment and intervention skills including therapeutic skills as well as skills in crisis management, loss and bereavement, parent work and other mental health interventions.
  • Preference will be given to candidates with knowledge and experience with an evidence-based therapeutic modality such as MI, EFT, CBT, DBT or ACT with adolescents.
  • Knowledge and experience working with high-risk youth trauma informed, harm reduction approach to care
  • Well-developed strengths in advocacy and collaboration.
  • Knowledge of the network of community services and resources; an understanding of health care delivery systems and the practice of social work in a secondary setting is also important.
  • Ability to collaborate effectively as a member of the health care team and to take initiative and leadership when appropriate.
  • Commitment to remain abreast of current literature and relevant developments in clinical practice.
  • Excellent interpersonal, communication and organizational skills.
  • Demonstrated ability to utilize critical thinking and problem-solving skills.
  • Demonstrated attention to detail and efficient time management.
  • Demonstrated ability to function as a member of the interdisciplinary health care team.
  • Demonstrated belief in lifelong learning and evidence-based practice.
  • Understand importance and demonstrates ability to foster a culture of Equity, Diversity, and Inclusion
  • Position is remote and would require a private space for sessions, there may be times when on site at the hospital is required, on site space is available if required.
