Applications are being accepted for a Patient Advocate. The successful applicant will be required to work days Monday-Friday. The commitment to this position is 37.50 hours per bi-weekly pay period.

ESSENTIAL JOB FUNCTIONS

  • Adhere to policies and procedures established for the assigned department while supporting the Mission, Vision and Values of Erie Shores HealthCare.
  • Serve as the central point of contact for patients and families seeking assistance, clarification, and/or support.
  • Provide a process for conflict resolution that supports patients, their families and our staff in a just, equitable and empathetic manner.
  • Demonstrate flexibility by navigating and managing time in both a fast-paced environment like the emergency department and inpatient units.
  • Assist patients and families through their healthcare journey and connect them to available community resources for continued care and support post discharge.
  • Provide emotional support, crisis intervention, and navigation assistance using a trauma-informed, culturally sensitive approach.
  • Participate and lead quality improvement projects aimed at enhancing the patient experience.
  • Lead reporting of reports, metrics, and summaries for senior leadership, Patient Experience Committees, and the Board as required.
  • Maintain accurate, confidential, and complete documentation of interactions in the electronic health record.
  • Manage patient feedback incident reports in a timely, collaborative and patient-focused manner, adhering to provincial standards and best practice for timely response to families/patients.
  • Collaborate closely with clinical leaders, physicians, and interprofessional teams to resolve issues in a timely, fair, and transparent manner.
  • Collaborate closely with access and flow teams to navigate challenging discharges understanding the patient/family perspective from a trauma focused approach.

QUALIFICATIONS

  • Bachelor of Social Work (BSW) or a Bachelor's degree in a health-related field is preferred.
  • Additional training in crisis intervention, conflict resolution, or trauma-informed care an asset.

EXPERIENCE

  • Minimum 5 years of experience in a patient-facing or advocacy role within healthcare, social services, or community support
  • Experience supporting vulnerable populations, navigating systems, and addressing complex psychosocial needs.
  • Demonstrated experience de-escalating conflict and guiding difficult conversations.
  • Exceptional interpersonal communication, empathy, and active-listening skills.
  • Strong critical thinking and problem-solving abilities.
  • Knowledge of hospital processes, community resources, and healthcare legislation.
  • Proven self-starter with strong multi-tasking and organization skills.
  • Detail-oriented with a high level of accuracy, and a proven track record of project follow-through.
  • Bilingual/Multilingualism is considered an asset
