Reporting to the Patient Flow Director, the Patient Flow Coordinator plays a key role between Providence Care's staff, physicians and external partners in ensuring seamless, quality and efficient patient flow activities. The Coordinator responds to inquiries from key stakeholders directly and/or indirectly and assists the Manager by researching and providing requested data or background information, as requested. The Coordinator problem solves barriers to transitions in care by working closely with the Access to Care and Transition and the clinical program teams to identify solutions. The Coordinator completes daily, weekly and monthly summaries provided to relevant 'leaders in the hospital and is fully informed of issues and challenges that require action.

Skills And Qualifications Required

  • Baccalaureate degree in Nursing, preferred or an equivalent combination of education and experience
  • Master's degree in health studies from a recognized educational institution, preferred
  • Current, unrestricted registration with the College of Nurses of Ontario
  • Two years of experience in providing clinical practice in a hospital setting with the relevant population. Intake and discharge planning functions preferred
  • Experience with electronic patient record systems
  • Valid Ontario Driver's license, clear Driver's Abstract and access to a reliable vehicle as necessary

Position Responsibilities

  • Leads Bed Utilization Meetings and other key meetings, as assigned, to facilitate communication and coordination related to patient flow
  • Reviews Daily Dashboard and clarifies data quality issues
  • Problem solves barriers to transitions in care at admission, transfer and discharge
  • Provides daily and weekly summaries to Providence Care leadership
  • Review trends, variances and reasons for variances for admissions, transfers and discharges
  • Provides real-time resolution of issues brought forward regarding barriers to admissions, discharge refusals, repatriations and community support services
  • Point person for relevant service criteria, patient flow and transition and patient/family inquires
  • Provides leadership in care navigation and discharge planning by collaborating with the client/patient/family and assisting the client/patient in the development of a discharge plan consistent with realistic, achievable and identified goals
  • Takes a primary role in managing patient/client/family/provider expectations about discharge and communicating discharge logistics to achieve timely transitions, positive outcomes, service efficiency and effectiveness
  • Facilitates access to care and transitions by actively supporting predictive discharge planning, where applicable
  • Acts as liaison and assists clients and staff with coordination of Home and Community Care and other community resources/services
  • Builds and maintains linkages with appropriate community agencies to facilitate effective collaboration related to access to care and transitions
