Social Service Worker

The Case Worker is responsible for providing supervision to the residents and coordination of activities in a safe, secure environment at the addictions extended care facility for men. The Case Worker is also involved with clients that attend the day program called the Community Outreach Program (COP).
Duties:
- Conducting case management assessments, case plan reviews and discharges, and when appropriate, make referrals to referral agencies and other agencies involved in the clients cases;
- Ensuring residents are admitted according to policy and orientated to the facility;
- Providing individual counseling to residents to assist in the recovery plan, independent living skills, stress management and problem solving to enhance the ability to deal with the consequences of addiction;
- Facilitating/Co-facilitating psycho-educational groups;
- Conducting regular checks of the house and residents, ensuring safety/security for residents and staff by screening visitors and enforcing visiting policies;
- Reporting/documenting crisis situations, i.e., breach of program guidelines or violation of agency policies;
- Assisting residents with medications as prescribed by observing the process and ensuring the resident signs that medication has been taken;
- Maintaining documentation and case notes, and recording daily progress notes and summary log book;
- Assisting the supervisor with planning/developing and evaluating programs and sessions offered to residents;
- Performing other related duties as assigned by the House Supervisor (i.e. assist with referral process);
- Ensure visitors to the House are appropriate and timely;
- Have knowledge of the fire plan and ensure clients are orientated to the plan;
- Review Policy and Procedure Manual regularly to maintain knowledge and understanding of protocol;
- Ensure policies, procedures and guidelines are adhered to by clients and staff;
- Deal with emergency situations immediately and document as required;
- Monitor physical condition of clients;
Minimum Qualifications:
- Successful completion of a Bachelor Degree in Psychology, Social Work or a related field with experience in mental health or addictions, OR a 2 year related diploma program supplemented by extension experience in the mental health and addictions field.
- Possess a solid foundational knowledge of addiction treatment theory and the recovery process.
- Current certification in CPR, First Aid and Non-Violent Crisis Invention.
- Experience conducting education/therapy sessions either one on one or in groups.
- Have good communication skills both verbal and written.
- Demonstrated interpersonal skills and ability to work independently.
- Excellent problem solving, conflict resolution and facilitation skills.
- In the interest of patient care, this position is open to male applicants or those identifying as such.
Other Qualifications:
- Basic computer skills would be considered an asset.
- Certified in acupuncture and training in motivational interviewing would be considered assets.
- Training and experience in adult education would be an asset.
- Additional relevant education and experience will be considered an asset.

Salary Range:
$ $35.41 per hour (Level 13 - UPSE)
Bi-Weekly Hours:
75.0 Hours Bi-Weekly / 8 & 12-hour shifts / Days / Evenings / Nights / Every Other Weekend
