The Hamilton-Wentworth Chapter of Native Women's Inc* is currently accepting applications for the position of Transitional Support Worker.

DUTIES:

The Transitional Support Worker, under the direction of the Executive Director, will provide physical, mental, emotional, and spiritual case management and support to Indigenous women and children residing in the transitional housing program.

RESPONSIBILITIES:

  • Provide housing supports, resources, and services to NWC program participants and be a community support for referrals from other agencies supporting Indigenous women and children.
  • Conducting entry assessments to shelter, intakes, goal planning, case management/meetings, individual and group programming, discharge planning.
  • Engaging with clients and community members in a respectful, non-judgmental and supportive way that helps to improve their health and well-being, develop life skills, increase their sense of belonging, and build resilience;
  • Incorporate Indigenous Culture as a way of being and knowing in the organization;
  • Complete intakes, safety plans, case notes, incident reports, plans of care, etc. within transitional housing environment.
  • Engagement with clients in a respectful, supportive way that helps incorporate culture as a way of being and knowing to improve their health and wellbeing, develop life skills, increase their sense of self, belongings and build resilience.

QUALIFICATIONS:

  • College diploma in social services or closely related field and a minimum of 3 years' experience in associated field;
  • Knowledge of the homeless and housing sectors in the Greater Hamilton Area, including resources;
  • Knowledge of and experience working with a range of issues impacting youth, women and children including homelessness, violence against women and children, human trafficking, poverty, mental health and substance use.
  • Experience working in an Indigenous organization is an asset;
  • Demonstrated experience in case management supporting people with diverse needs;
  • Ability to advocate on behalf of clients to facilitate access to identified supports;
  • Intermediate computer literacy skills including Microsoft office suite and databases;
  • Demonstrates cultural competency including awareness, knowledge and understanding of the impacts of colonialism;
  • First Aid / CPR /AED Certification
  • Non-violent Crisis Intervention and Trauma Informed Training;
  • ASIST Certification
  • Food Handlers Certification
  • Must have or obtain, within 30 days of hiring date, a Vulnerable Sector CPIC.

SALARY: $24.00/hr

JOB TYPE: 40 hours per week (Monday to Friday, 4pm to 12am).
