Position Purpose
As a key member of the program team, the Client Case Worker's purpose is to lead the case planning process, assist program participants to achieve and maintain stability in their housing, enhance access to community-based supports, services, and information; and strengthen and foster each participants ability to live within the program and wider community through a variety of educational and mentoring opportunities.
Key Responsibilities
- Assesses client's needs and risks using inventories, checklists, and questionnaires.
- Leads the case planning process, prepares case histories and case plans, and reviews care plans created by others to ensure compliance with ICCS care plan policies and protocols.
- Use their education and experience to lead the development of written care plans, recommend strategies of care, and remind and educate staff on the centrality of care plans in program culture.
- Ensures care plans are created in conjunction with caregivers and/or other service providers, and include needs identification, goals, tasks and timeframes with regard to client programs,
- Interviews clients and their teams to assess the impact of care plans and make adjustments to address changes in client diagnoses, health, and goals.
- Leads the development and communication of client-centered and client-specific guidelines for the program team to achieve client care outcomes.
- Provides information on and referral to other community service providers, resources and professionals as required.
- Models and trains clients in life skills and activities of daily living such as meal preparation and basic cooking, personal care, finances and money management, de-cluttering and housekeeping, shopping, household safety, and pet care in accordance with client's care plans.
- Supports clients to learn new social skills and function in individual or group settings by providing feedback, problem solving, and behavioural management options and strategies.
- Provides emotional support and assisted clients to learn and practice emotional regulation.
- Coordinates social and recreational activities within the program.
- Recognizes, analyzes, and deals with potential problems and emergency situations such as clients' aggressive behaviour to ensure no harm comes to the client and/or the public. Reports problems and emergencies to the supervisor and completes incident reports.
- Identifies recreational, educational, social, and vocational services and activities that will meet clients' needs and encourages and supports clients to attend or participate.
- Transports and assists residents to appointments, shopping, or leisure activities.
- Gathers information relevant to the client's care through case conferences, client interviews, and observation to develop and implement intervention plans within program guidelines.
- Provides input to the program manager and integrated care team partners regarding program changes, supports, and improvements that could assist clients to achieve their care plan objectives.
- Liaises with other agencies, professionals, and the community.
- Provides evaluations and reports of client's progress, and contributes to and maintains client records, statistics, and logbooks.
- Performs other related duties as required.
Qualifications
Education
- Degree in Social Services or equivalent education/experience.
Training and Experience
- Two (2) years experience or an equivalent combination of education, training, and experience.
Required Licenses/Certificates:
- Class 5 Driver's License & Driver's Abstract; Business Class Insurance
- Basic First Aid with CPR-C
- Naloxone Administration
- WHMIS 2015
- Non-Violent Crisis Prevention
- Successful Criminal Record Review for Vulnerable people
