The West Ottawa Homeless and Addiction Recovery Treatment (HART) Hub (WOHH) is a new collaborative initiative being driven by seven core partners and led by PQCHC, with support from more than 20 organizations across the region. The HART Hub will provide safe, low-barrier and judgement-free access to a full continuum of services for people in need of support with their substance use health. This continuum will include primary care, addiction medicine, withdrawal and treatment services as well as housing support, peer connection, and access to social services. The West Ottawa HART Hub will operate out of two locations- a main location housing a walk-in clinic and withdrawal management and a secondary location housing the bed-based stabilization and treatment services.

Job Summary:

The Physician Assistant (PA) is an integral member of the West Ottawa HART Hub clinical program, and in collaboration with the multidisciplinary team, will provide a wide range of services under the supervision of licensed physicians including comprehensive triage, medical assessment, diagnosis, crisis support and treatment to patients with varied health, social, cultural, and economic barriers. The focus will be in the provision of primary care, mental health, infectious disease treatment, substance use management and harm reduction.

With the team, the PA will work within a trauma-informed, harm reduction, culturally safe and patient-centered framework.

Job Specific Responsibilities

Direct Client Service

  • Provides clinical services to clients, either in the Hub or in the community, taking into account the environmental, social, psychological and medical factors that influence health within the scope of their professional license.
  • Provides individual assessment, education and outreach to individuals with substance use challenges and related wraparound care needs. Assesses, diagnoses and prescribes treatments to clients, and refers to other resources as appropriate.
  • Participates in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs and services for individuals, families and the community as they relate to the Hub.
  • Plans and participates in outreach health services.
  • Maintains awareness of current issues and developments in the respective field.
  • Performs health promotion activities on relevant topics.

Administration

  • Maintains electronic/paper client records and protects the confidentiality of the information.
  • Performs administrative functions as required by the programs.
  • Assists in maintaining the program database by entering data, generating reports, supporting analysis, and regular auditing of data integrity.
  • Assists in preparation of program and promotional materials and other documents as assigned.
  • Maintain an inventory of program resources available to clients.
  • Participates in regular chart audits and reviews.

Teamwork and Collaboration

  • Assists in the development of program specific policies and procedures/guidelines and in the overall planning and development of the programs
  • Attends and participates at team and general staff meetings as scheduled
  • Participates in the evaluation component of relevant program
  • Maintains effective communication of information among internal and external stakeholders
  • Participate in staff development as appropriate to the position.
  • Works within established policies of off-site organizations where staff are housed.

Qualifications

  • Graduate of a bachelor or master's level physician assistant training program in Canada or the United States.
  • Registration as a Physician Assistant in good standing with the College of Physicians and Surgeons of Ontario, or eligible for registration.
  • Experience working in the field of primary care, mental health or addictions medicine – preferred.
  • Experience working with vulnerable populations experiencing various social, economic and health-related challenges – preferred.
  • Understanding of high priority communities and their health challenges.
  • Proficiency in the use of computers and various software applications.
  • Fluency in English essential, other languages desirable.
