The Opportunity
The Pharmacist monitors and evaluates all aspects of drug use within the Hospital and provides clinical pharmacy advice and support to physicians, nursing staff, healthcare professionals, and patients.

Qualifications

• B.Sc Pharm
• Pharmacy Examining Board of Canada (PEBC) + Jurisprudence Exams – Licensure with Ontario College of Pharmacists
• Must strive to continuously provide a safe and supportive environment for patients, staff, physicians, and volunteers
• Must adhere to the duties of workers, as stipulated in the Occupational Health and Safety Act
• Must have an ability to effectively communicate (written and verbal) in the English language in order to provide excellent communication with team members, patients and families. Ability to communicate in additional language(s) is considered an asset.
• Must have basic computer skills
• Must be dedicated to professional growth & development
• Must be able to perform all bona fide and essential duties of the position, which may include physical demands such as moving/transferring patients, pushing, pulling, bending, etc.
• Must have demonstrated excellent attendance/performance record
• Excellent organization and prioritization skills
• Ability to work independently and effectively communicate with other members of the health care team

Preferred

• Experience with Unit Dose Dispensing Systems, Antimicrobial Stewardship, Meditech an asset

Duties And Responsibilities

• Evaluates physician orders for appropriateness of dose, frequency, method of administration, potential interactions, therapeutic duplication, allergic reactions and contra-indications
• Ensures that non-formulary medications ordered by physicians are obtained from patient or supplied by hospital, or provides physician with recommendation for substitution with formulary drug
• Provides drug information and in-services to Physicians, Nursing, Medical Staff, and patients
• Oversees therapeutic drug monitoring of appropriate medications
• Identifies potential areas of improvement of hospital's pharmacy services, assists in the formulation and implementation of solution
• Promotes patient safety and staff safety in the workplace
• Antimicrobial Stewardship audits, VTE Prophylaxis audits
• Perform any other duties as assigned

Hours of Work:
Days, evenings, and weekends. Please be reminded that as a Hospital, in order to provide the necessary patient care, our operations run 24/7. Therefore, depending on the program for which you are applying, it may be an expectation that you are available to work weekdays and weekends, as well as all shifts - days, evenings, and nights.

Wage:
$ $66.99 per hour plus applicable shift premiums for evenings, nights and weekends.
